Question: Hello doc, my son is 22 months old and is not able to walk independently. His muscle tone seems to be weak and also drags his right foot sometimes while walking. Is it a cause for concern?

Brief Answer:
He needs complete evaluation

Detailed Answer:
Hi
Welcome to the HCM

Your child is delayed for his gross motor skills. Don't worry as many of the conditions are treatable. But before any such treatment he needs to be clearly investigated and diagnosed for the cause for his delayed walking.

I would like to know the following details for a better advice:
- he was born at term or prematurely
- did he have any significant complications or hospital stay especially at birth
- Were there any maternal illnesses during pregnancy
- Any significant family history of illness
- Is he normal is other fields of development I.e. fine motor skills, language, social, cognition
- any other medical problems

Kindly reply back so that I may help you in the best possible way.
Take care

Hello doc, Thank you, here are the answers

He was born after full term and not prematurely.
He did not have any complications or hospital stay so far.
My wife had post delivery depression and continues to suffer.
No other family history of illness
He is absolutely normal when it comes to other motor skills, language and is a very intelligent child. He however is suffering from GERD and acidity and his food intake is severely hampered. We are worried his nutrition is not upto the mark. He also has stranger anxiety and gets very much out of control when we visit a doc. He cried unctrollably and throws up what he has eaten.

Our worry is, although he walks, he trips easily and needs someone to hold his hands. He hain't got full control and his legs seem to be very thin.

Brief Answer:
The problem is most likely nutrition related but need complete evaluation

Detailed Answer:
Hi
Although from your description the problem seems to be poor nutrition, but he needs complete evaluation. So, I would recommend you to go for the following investigations:
1. CBC, CPK
2. S. calcium, phosphorus, Magnesium, Alkaline phosphatase
3. Intact PTH, TSH, free T4 and T3, 25(OH) D3
4. X ray knee joints

You may post the reports for my reference. Also, tell me about the GERD investigation done, treatment which he followed and the present dietary habits.

This will help me to suggest the best possible GERD management and dietary modification needed.
