Suggest Treatment For Urinary Tract Infection

Question: Urinary Tract Infection
I was down with viral fever for 3 -4 days, along with which I also developed a Urinary Tract Infection .
Medicines taken for viral fever :
Amoxicillin 500mg thrice a day for 2 days
Crocin SOS
B- complex One per day
I had also taken ORS solution, Glucose C
The fever came down to 99 degrees but there was extreme dehydration, loose motions leading to excessive weakness

I am a diabetic and my sugar was uncontrolled. On 19th August 201, I did a random test & the result was 220 & I immediately consulted a doctor
The doctor then changed my medicine, switched me to Insulin for the time being & advised to undergo certain blood test. This is the first time that I have been advised to take insulin.

Urine culture showed Klebsiella spp. With a colony count >100000/cc.
This was urine sample given before starting the treatment advised by the doctor for viral & UTI

On 22nd August 2014, I again checked for Sugar Fasting and it came out to be 221
Doctor then increased the dose of insulin from 14units to 20 units before breakfast & from 10 units to 16 units before dinner

Current treatment as below
INJ.MIXTARD (30/70) 20 units before breakfast & 16 units before dinner
Tablets AMARYL 1MG before breakfast / Ceftum 500mg twice a day/ Niftran 100 mg twice a day / Pyridium 200mg twice a day

Now today on 25th August 2014, again I checked my sugar levels Fasting is 151 & PP is 322.
The doctor has again advised to increase the Insulin dose
INJ.MIXTARD (30/70) 28 units before breakfast & 20 units before dinner and also TAB AMARYL M1 before breakfast

Hospitalization was an option given to me by the doctor but I preferred to be treated at home & if still not controlled would get admitted
Though my doctor has advised me it will take up to 2 weeks for me to be fully cured but I’m concerned if Hospitalization is mandatory to get rid of the infection & sugar to be controlled subsequently and if there is a possibility of Diabetic ketoacidosis or any other complication ? Is the treatment given to me by my doctor the right treatment ?

The earlier followed prescription for diabetes & blood pressure is attached, please refer picture P4b

-for better control of infection by injectable antibiotics.
-for better monitoring of sugar levels before each meals and give insulin accordingly.
-if you go into any complications they can be diagnosed and managed quickly.

The treatment your doctor advised is correct . That is the usual protocol .

Your sugar levels are not getting controlled because of your UTI(Urinary tract infection) and viral infection. UTI also is not mild. Bacteria load is more.

Yes, there is increased risk of you going into ketoacidosis because you have dehydration, infection and high sugar levels.

HI DOCTOR,

I have gotten admitted in the hospital now. An ultrasound was performed and in the same it was discovered that I have stones in gall bladder as well.
I have attached the required reports. Can you please tell me the following :
1. Has the UTI infection been cured as per the reports ?
2. What is an advisable procedure to cure the stones - Complete Gall bladder removal or only gall bladder stones removal.
Can the stones also be dissolved through medicines.
3. My sugar level is fluctuating as of now fasting at times the fasting is coming 151 , 171 and random done before meals is sometimes 200, 288, is it advisable to go for gall bladder surgery with fluctuating sugar levels. I am still in the hospital

Hi Mam,

Depending upon your ultrasound and blood counts it appears that your UTI has cured. But you should have relief from symptoms also.

For gall bladder stone treatment is removal of gall bladder. Because
-gall bladder is diseased . If left by only removing stones may cause recurrent stones.
-in diabetic patients with multiple stones its better to remove gall bladder because chances of complications are more. It is unlikely that with medications stones dissolve. Chances of forming again will be more.

Till your sugar levels are not controlled surgery can't be done . As healing and infection problems may be there and more of post operative complications . I suggest you to control sugar first and then get surgery done .
