Brief Answer:
You need to evaluate and decide about modality of treatment .

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that you have Enlarged Prostate with diabetes and hypertension and having problem of urgency with incontinence of urine .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) What is the size of the prostate reported on Ultrasound Scanning ?
2) What is the nocturnal frequency for urination ?
3) Have you done Uroflometry if so what is the average flow rate ?
4) Is your diabetes under control ?

Urgency with incontinence even with taking Solifenacin and Alfoo need to evaluate and change the modality of treatment .

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Uroflowmetry.

Medical treatment in such a situation has limited results and one has to take medication till one decides to get operated

You can discuss and explore the feasibility of an endoscopic resection of prostate by a surgical procedure called TURP .

This is a gold standard method to resect the enlarged prostate and improves quality of life for forever .

Hope I have answered your query ,please feel free to ask if you have more questions , I shall be happy to help you .
Thanks and Regards.
