Question: Dear doctor
I am from XXXXXXX XXXXXXX XXXXXXX
Since long I am suffering from some mixed ill feelings and the actual cause is not diagnosed yet. Basically I am a lazy person and I am a responsible govt servant and I do lot of computer works.
Since childhood my stomach is not healthy and I have bowel uncertainity.Sudden latrine tendency remains all along.
Since last 15 years suddenly I found numbness in my scalp and head had less concentration.I could not concentrate and head becomes puzzling in sorting ot things and arranging the sequence in order. Eyes were irritating. Less vision pleasure. Water secretion from both eyes. Cannot see an object constantly. Felt shock in sudden heavy sound around and electrifying fear at the high speed vehicle passing nearby etc. I thought it was a brain tumour and I insisted on CT scan. Nothing was found and it was ok. Then I did MRI of neck and head. Nothing significant found.
Slowly my feet and hands felt numbness and no energy in them. Could not keep hands and feet raising for long due to numbness. Could not go walking a tall building staircase due to numbness in feet.
Then came another symtomps. My chest part was paining at different locations. Feeling very weak Heart and lungs. Shortness of breathing. But if I burp artificially it gives me a bit relief in chest. In that moment I cannot concentrate any sound, talk, looking and deep fatigue feeling is found. Eyes becomes tired and exhausted.
Sometimes I feel like vertigo and fainting kind of feelings. I always have a fear of sudden extremes to me.
Since then I kept on doing ECG, treadmill for heart. It was found ok. Vit B12 deficiency test was ok. My heartbeat remains faster and doctor prescribed me Embeta XR 25. My cholestrol and triglyceride remains high and doctor gives me Tonnact TG.
Rest for gas and stomach doc gives me Rabikind and related drugs. For relaxation doc gives me Zapiz 0.25.
My uric acid remains high and I dont take any regular medication for that. My sgot sgpt is normal. Creatinine is normal. Sugar is normal. BP is within normal
I dont have energy and I have continous mental and physical fatigue. No energy to walk or go to gym.

Doctor kindly diagnose me and suggest.

Pertaining to your bowel symptoms ,it possibly seem to have Irritable Bowel syndrome.

This IBS picture can be corrected through both drugs and lifestyle friendly measures.

IBS is associated with underlying stress and anxiety.

Relaxation techniques such as meditation,
Physical activities such as yoga
Regular exercise such as swimming, walking, or running are known to be effective against IBS

Use probiotics like Florastor one capsule a day for 3 months.

Probiotics have positive effects such as enhancing the intestinal mucosal barrier.

Probiotics may also have positive effects on the gut-brain axis by their positive effects countering the effects of stress on gut immunity and gut

Pertaining to your eye symptoms,you probably have computer vision syndrome/CVS.This symptom complex can cause all your head and eyes.

Computer vision syndrome (CVS) is a temporary condition resulting from focusing the eyes on a computer display for protracted, uninterrupted periods of time.

"20 20 20 rule" can be adopted to minimize symptoms of CVS-every 20 mins, focus the eyes on an object 20 feet (6 meters) away for 20 seconds.

This basically gives a convenient distance and time frame for a person to relax his eyes.

Decreased focusing capability is mitigated by wearing a small plus-powered (+1.00 to +1.50) over-the-counter pair of eyeglasses. Wearing these eyeglasses helps such patients regain their ability to focus on near objects.

Since the brain and neck scan/s are normal ,you need worry much about this .

Pertaining to the chest and bodily symptoms ,many of them are related to mixed picture of anxiety neurosis and Hyperuricemia(elevated uric acid)

But having said that even elevated uric acid levels/hyperuricemia can cause such symptoms.

Since the cardiac work up is normal,you need not think on lines of a heart disease.

Elevated heart beat will subside with stress management.
A short trial of drugs like Clonazepam 0.25 mg once daily at bedtime for 3 months will be effective.

Continue all the prescribed medications as suggested by your doctor.

Elevated uric acid levels usually does not need treatment if not producing any symptoms.

But I advise you the following measures to lower your uric acid levels to improve your symptoms.Medications are needed only if symptoms does not resolve with these lifestyle modifications.

Consume foods that are low in purine like:
Refined cereals - breads, pasta, flour, tapioca, cakes
Milk and milk products, eggs
Lettuce, tomatoes, green vegetables
Cream soups without meat stock
Water, fruit juice, carbonated drinks
Peanut butter, fruits and nuts.

2.Keep well hydrated, drinking 2 to 3 liters of water per day.

3.Avoid caffeine and alcohol, as these can contribute to problems with uric acid and hyperuricemia.

4.Avoid medications that can aggravate uric acid levels.

If you experience symptoms or side effects, especially if severe, be sure to discuss them with your health care team. They can prescribe medications and/or offer other suggestions that are effective in managing such problems.

Thank you so much for your kind and elaborated reply.

However, I am yet to know the reason for the numbness of my legs and hands. Further,sometimes my left hand feels some pain and numbness. I also forgot to tell you that I have been using spectacle with both + ve and - ve powers accompanied with cylindrical power.

I also forgot to tell you that often I feel pain on my toes due to uric acid with very rare and less swellings

Further,I am not clear about my vertigo type problems. Sometimes I feel imbalanced while crossing roads.

Sir,kindly diagnose the above and advise me about any new clinical tests are to be done so that further clarifications can be made.

Foot pain related to hyperuricemia.

Hi Sir,

I understand your concern.

Cylindrical lens seems okay at the moment.Get your vision tested every 6 months.

Pertaining to the numbness along with pain in the extremities,these are directly related to the effects of hyperuricemia.

Hyperuricemia does not always present with swellings.It can initially manifest with pain alone.

Regarding Vertigo,initially I advise you to go for serum B12 testing,since this vitamin deficiency can cause such manifestations.Based on the test result,treatment is directed appropriately.

Further tests like MRI brain may be needed if this turns B12 out normal.
