Suggest Treatment For Throat Infection After Sexual Activity

Question: Hi Doctor, I started having throat pain around 4 months back (around 10thMay'2015). Initially one week I thought it would subside by itself but symptoms got more prominent such as persistent throat pain, bit of hoarse voice, tendency to clear throat frequently BUT NO FEVER. I Went to ENT doctor who did physical examination of my throat and Diagnosed granular pharyngitis. He gave me tab claribid (500) 7 days antibiotic, tab xyzal m (continuing 1 daily for last 4 months) and cap mucolite for 15 days. He also gave me Rabicip(20) for 15 days approx. for acid reflux along with daily saline gargles. I took the treatment for 15 days, however no significant improvement. Than I again went to ENT because I did not have much improvement. He got my NCCT PARANASAL SINUSES done which came normal -

ENT doctor told me that It is due to allergy, however I neither smoke, nor take any cold fluid, cold water, spicy food, take lot of precaution and selection in what I am eating.

I was simply kept of anti allergic xyzal m for one month.

Than in july problem got more severe I was having difficulty swallowing, could not eat solid food, also after brushing trying to clear my throat I was getting slight blood stains. I again went to the ENT and told him all this. He got my Barium Swallow test done, along with CBC, ESR . All reports CBC,ESR and Barium swallow were normal. Since my symptoms were painful that time, he again gave me antibiotic CLARIWIN (500) this time and along with same anti allergic XYZAL M. This antibiotic subsided my symptoms a bit and I atleast starting eating food with lesser difficulty. Although symptoms were still quite prominent, only slightly better.

After that Doctor said it is your allergy and it might take months. Post july antibiotic second round, I am only taking Xyzal m daily. However I still have sore thorat, slight difficulty while swallowing, I can see some white yello spots/ patches in my thorat around pharynx and tonsil area. Lately a new finding happened, My wife was having some gynaec itching problem for quite few months and lately went to Gynae doctor who did all the tests on her and she was found with E COLI bacteria urine infections. In last 4 months I has oral sex with her few times and can recall with lot of confidence that my first throat problem started after the oral sex 4 months before, I many times got doubt about this but never discussed this with doctor. Gynaec doctor also asked me to take a AF-KIT (pack of 1 fluconazole, one azithromycin,2omidazole). After taking this 1 pack surprisingly for next 2 days I felt like magic has happened to my throat and did not felt even 1 percent symptom of any throat pain, problem etc. But after 3-4 days situation got back to the same position. I have same persistent throat pain, difficulty while swallowing. In these last 4 months of throat problem I have never had any FEVER or any other problem.

Now I doubt that my throat infection has been due to the oral sex and may have been passed from my wife. But I am not sure if it E COLI or any other bacteria, Virus or Fungus. But I never told this to my ENT doctor.

I am not sure hot to get rid of this throat problem as it is creating lot of problems to lead a normal life. Please advise further coarse of treatment, any test required (Such as Throat swab), What antibiotics if it due to bacteria or any anti fungal medication, if it is due to fungus. But one thing is certain AF KIT did magic in my throat for 2 days which means some tablet was working, may be I have to take it for longer time to completely get rid of the bacteria, fungus or virus. Please advise

Thanks for your kind assistance and Apologies for long post. But I need your help doctor as I am fed up with this throat problem and every time I go to my ENT, he simply says it is allergy and will take time, But I doubt this is some infection but we just need to find right root cause to eradicate it with right medicine.

1. Get multiple throat swabs done. Request for MIC (Minimum Inhibitory Concentration) values for the antibiotic sensitivity. Send one swab for fungal culture.

2. It is also possible to get E. Coli infections from other sources. You must inform your ENT Specialist. E. Coli is notorious in helping other bacteria also get resistant.

3. Multiple infections are common. Additional antibiotics suchas ciprofloxacin or intravenous antibiotics may be required. A medicated mouthwash such as Betadine should be started.

4. Treatment of your partner/spouse is also recommended.
