Suggest Treatment For Testicular Lump

Posted on Thu, 24 Dec 2015
Question: I am a 20 year old male, recently while doing a tse i found a painful hard lump at the top of my right testis seemingly under the epidermis(however attached to the actual testes), any time i touch it pain radiates through my right side or low abdomen. If I don't touch it, it doesn't hurt. Due to severe anxiety I have squeezed the lump everyday, despite the pain. In the past 2 days I have notice the testis is softer than normal, (the bump is still there).I have made an appoint for 12/10/15 at my university medical center. What conditions/disease could cause this? What is the likeliest cause? Thank you in advance....

Brief Answer:
Cyst can be the most probable reason.

Detailed Answer:
Hello!

Thank you for the query.

Most likely such lump is a cyst of this area. Cyst is a benign lesion filled with the fluid. Sometimes it can be hard when palpating it.
Other possible reasons are inflammation and varices. The worst possible reason is a cancer (however it is not very probable).

Ultrasound is the tool which can answer what exactly are you dealing with.

Hope this will help. Feel free to ask further questions.
Regards.

Thank you for your response....! i would like to add that it is not on top but near the top, does this change the most likely probable cause? When I squeeze it the pain is mostly in my right side/abdomen......

Brief Answer:
Cyst is still most probable.

Detailed Answer:
Squeezing it you also squeeze epididymis and spermatic cord. Even if there was no lump in this location, squeezing would cause pain which radiates to the abdomen (spermatic cord comes from the abdomen). So this does not tell anything about the lesion.
Location also does not tell much as the testicle is XXXXXXX Top or near the top still makes the cyst most probable.

Thank you for your previous answers they have been very insightful. I have one more follow up question.... After urinating in the morning I notice that the lump/bump seems to have softer , as if it was less filled, however my testicle seems to feel like its normal consistency however it is swollen, what does this mean? Does it have anything to do with squeezing the bump?
Just to add I am now able to pinch and feel space between the bump/lump and my testicle.....

Brief Answer:
Spermatocele.

Detailed Answer:
Urinating has nothing to do with testis and scrotum. Urine does not flow through spermatic cord, groin or testis. So there can not be any connection between the urinating and lump consistency.
It may become softer when you squeeze if a part of fluid gets out through spermatic cord and urinary tract.
This makes epidydymis cyst (spermatocele) most probable diagnosis.

As it is not attached to the testicle, testicle cancer is hardly probable.
