Question: My mother is 89 and 4 dats ago she had 3 large bites on her back that became very swollen and painful, now she has a large red burning rash under one arm, could this be lyme?

Based on your history and the image uploaded this rash is not the erythema migrans rash associated with lyme disease.

If the area is not endemic and the rash doesn't appear to be that of lyme then treating Lyme empirically may not be justified.
