Question: Hi Doctor, i have swelling on tip on my urinal and its has scars too, it started with itching a week ago, it paining when i touch it or trying to wash it i have attached photos

“As” per your image is concerned it seems like fungal infection and it is a common infection you can use OTC anti fungal cream [clotrimazole 2%] for few days and wash the area in diluted apple cider vinegar solution. To prevent it in future you can use loose garments at night and every time wash and dry it after urination.
