Question: Thought I had a groin pull, but next day significant swelling and pain in left testicle, Groin pain subsides but hardly any change in swelling or pain in testicle, hernia ?

In fact as I was reading and analyzing your query
I was thinking about hernia

The hernia is a small sac like cavity that comes out of (commonly) the intestines
The tract may cause pain and if it goes into the scrotum, or pressurized the scrotal content, may cause pain there.

The rest may reduce the swelling but the pressure symptoms may remain.

Suggest a usg of the abdomen and scrotum
If found positive, you may need surgical repair
