Suggest Treatment For Stomach Pain, Gas And Loose Motion

Question: hello doctor,
I am suffering from stomach pain and loose motions. and Gas problem. when i was examine by doctor, she asked for stool and urine analysis. I did and got the report.
Stool :
pus-2-4
RBC-2-3
others- Yeast Cell (+)
consistency- soft
Urine :
reaction- acidic
pus cells - 5-7
others - mucus thread (+)

she gave me 4 medicines.
1 - omeparazole
2 - Tinidazole 500 mg(told take 4 tabs all togather)
3- Fluconazole 150mg
4- Duspamen (mebeverine HCI)

I am a little reactive with most of the medicines. I just want to know that did these medicines are suitable for me or not. I was epileptic before 5 years, and doctor told me not to take quinolones group of medicines.

Need your advice.

The medication your doctor prescribed are corrsct based on the symptoms you described and your lab reports. Quinolones are antibiotics that havent been prescribed. One can onlu document sensitivity to a medication if there has been an allergic response. Have you taken any of these medication previously and was there any adverse response/ reaction ? Thank you doctor,
But i never took these medication except omperazole. I just had my meal and should i take fasigyn tab only 1 tab. Because taking four tab alltogather is little weird..

Hello again.

All these medication are usually very well tolerated with minimal side effects in practical practice. You should take all 4 as the standard dosing in tinidazole is 2 g for most conditions. 500 mg would be too low.

hello doctor,

is it possible that due to gas, blood pressure can be high, because just now my blood pressure rise upto 160/110 and after some belching and gas pass its come down to 138/97

GERD and hypertension are two separate diagnoses. Obesity and stress is related to both and weight loss, stress reduction can benefit both conditions. A single reading should not be used to ascertain a high B.P. 24 hour ambulatory monitoring can be advised if the reading is persistently above 140/90.

Hello Doctor,
Last time you answered me about the BP and GERD. That day my BP goes high suddenly. I was just having gas and fullness of stomach. But after gas passing my BP goes normal and i slept normally and did my Office work normally. From that day i started checking my BP regularly and i find it normal.
I gave you my Medical Reports before that i suffered from stomach flu and i took med flucanazole. now i am taking only omeparazole two times daily with duspamen.
i m feeling pain around me right waist. my urines is little yellowish but no pain during urination. i am little araid, that waist pain is with burning sensation and it started suddenly and still i am belching because of gas.
please help me out.

Hello again.

Its likely due to GERD. Though you are taking omeprazole, in my clinical experience , combining it with pro kinetic like metoclopramide, antacids such as bismuth subsalicylate and sucralfate yields a much better result. If the pain persists or worsens then an EKG, serum amylase and ultrasound abdomen should be advised to look for other causes such as cholecystitis, pancreatitis etc. If despite adequate treatment the dyspepsia recurs then an upper G.I endoscopy could be advised in consultation with your doctor.

I have a few queries.

Have you been tested for H.Pylori ?
Have you started the tinidazole etc which you asked about in the previous queries ?
Did you experience any allergic response with these medication or did you tolerate them well ?
Have you had an ultrasound of the abdomen/pelvis recently ?

Yes i had a stool test for H.pyroli and it was negative.
and yes i took 4 tab of tinidazole. and yes i felt dizziness.
only ultrasound left to be diagnosed.

Tinidazole and fluconazole can also cause varying degree of G.I discomfort. However, the stool antigen is a very specific test for H.Pylori and makes it unlikely for H.Pylori. You should get the ultrasound done to look for any abnormality. You may get back to me with the results. For now you can continue the medication as explained in the answer above.

Hello Doc
I went to the doctor again.
He just asked my problem and gave me another flucanazole. with some menthol herbal pills for bloating. thats it. But i didnot took other flucanazole because it reacted a little before.
as of my stomach condition, my pain is not like before. it start paining when i eat something. I am eating a simple diet. since yesterday i found my stool changes in yellow peas shape. but i dont have pain now. but one weird things is when i done with my toilet i feel sudden pressure inside my body. i got sweaty and feel panic and anxiety. though i manage to control it by not thinking about this.

ryt now i m taking only omperazole with dompy in the morning and evening.
i need your suggestion. Doctors in saudi arabia are not good. (in my view).

Fluconazole is an anti fungal medication with very little reactivity. In my opinion you should take it as your stool exam showed evidence of fungal infestation.

Post defecation dizziness / heaviness / feeling of light headedness is quiet common and can occur especially with more straining and constipation. The circulatory changes take time for the blood to be distributed equally after the defecation. It usually reverses within 30 to 40 seconds.

Sometimes, a doctor may not communicate his/her ideas as well as one should. the medication prescribed are correct but I feel your doctor hasn't discussed with you in detail about your condition taking into account your apprehensions.

I would advise you to consult an FCPS holder in gastroenterology in Saudi Arabia. Its a recognized structured post graduate degree program for fellows here in Pakistan who also work in Saudi Arabia.
