Dr. Tayal . . . Thank you for once again addressing my situation. Quickly, I am on the Vitamin D3, 2,000 IU, for two weeks now. I do notice some improvement. I am trying to keep my shoulders limber but it is painful. I still have two painful shoulders but finally getting periods of relief off and on. It has always been both shoulders from the onset with the left shoulder having the most pain. Perhaps because I am right-handed and therefore use my right arm more. The pain continues in the same shoulder area and triggers some shooting pain down the outside upper arm for about four inches. My wrist pain is almost completely subsided which is a blessing. One thing that has me concerned -- I notice that my hand (left) wants to shake sometimes when my arm is not supported and I am attributing that to the shoulder pain (this is something new as of yesterday) The hand wants to shake when I hold my arm straight out, unsupported. My range of motion is fair, but for bringing my arms around to the back. I am unable to raise my hands even to my waist, but I keep trying. It is the shoulder pain that prevents me. I still believe your assessment of post viral arthralgia is correct. Next Sunday it will be four months. If I am not being patient enough, please tell me.

I appreciate and believe in you and I am so grateful for your concern and guidance. I am unable to turn to my family doctor since he changed affiliations and advice from friends and family have me stressed and concerned. They talk of RA flare, pinched nerve, a frozen shoulder that could develop. I discontinued the physical therapy because I am concerned that more harm than good could come from that due to not knowing what is wrong with both shoulders and the issue of post viral arthralgia, which seems to be unknown to them. Please know how much you are appreciated; please know that from me.

Thanks for posting your query.
It has been four months since the fever. The pain in the joints should have subsided by now. If you are unable to lift your arm above your head then frozen shoulder needs to be ruled out. The shaking of the hand can be due to muscle weakness as you are not able to use the arm properly. Physical therapy can help at this point in improving the range of movement. You ca also get a muscle relaxant prescribed from your physician and see if it provides a better relief.

Thank you, Dr.Tayal . . . In response please note that I AM ABLE to life my arm above my head it is just that there is a point of pain and once past that my arm will raise straight up, as my right arm does. There has been no fever for quite some time. You said the pain in my joints should have subsided by now. That concerns me because the pain has been so long now. Is there a clue in the fact that the shoulder pain is the same and in both shoulders? I can begin physical therapy again, I just don't know how to handle the 48 hours of unbelievable pain after a therapy treatment.

Start physical therapy if pain subsides with hot compress.

Detailed Answer:
If you can raise your arm above the shoulder level then frozen shoulder is not likely. The pain subsides 2-3 months after the fever subsides completely. Since the pain is same in both the shoulders, it is not likely to be a local shoulder related issue. Post viral arthralgia is most likely. Now that fever has subsided, the recovery will be faster.
You can start physical therapy after you get proper pain relief with hot compress and mild pain killers.

Thank you Dr.Tayal . . . I agree with the post viral arthralgia once again. I am having more days with tolerable or less pain in my shoulders but there are also some very painful days tucked in with them. I think I need to have more patience and realize that this will take time to subside. What I will do now is continue on the 2000 IU's daily of Vitamin D3, keep my shoulders, arms and wrists moving as much as possible, while realizing the shoulders are fatigued, and, taking a rest period each afternoon. I am finding that sleeping with more pillows and making sure my shoulders are supported through the night seems to be making a difference for the good. I don't feel ready to go back to physical therapy - the 48 hours that follow each session are just too painful to endure. On occasion I am taking Tylenol extra strength, 500 mgs, the Vitamin D3, daily, and a Centrum Silver daily, Multivitamin.
For your time, your sincere concern and your confidence in what is going on with my shoulders, arms and wrists, I am so grateful. It will be four months on this coming Sunday and I will have patience and stop with the stressing and fear. You have been my blessing Sir -- thank you so much. XXXX

It is best to continue with daily calcium and vitamin D3. Do a few exercises of shoulders and wrists at home as long as these do not cause you extra pain. It will subside gradually. Please do not worry.
