Splenic enlargement is associated with a number of conditions, most of which are infective in nature. There are several bacteria and viruses that could lead to splenomegaly (enlarged spleen) along with hepatomegaly (enlarged liver). So, when we see splenomegaly, we try to rule out such infections as a probable cause. Treatment of the underlying cause helps in its resolution.

Now coming to your questions:
1. Splenomegaly itself would not have the complications. But the underlying condition would. It would therefore depend entirely on the underlying condition which needs to be diagnosed.
2. Infective medicine specialist or simply a internal medicine specialist would be able to evaluate splenic enlargement. Hematological disorders can also lead to splenic enlargement but an internal medicine specialist would be able to identify the condition. If required, he may go for a hematological referral.
3. The answer is yes. However, the steps are not definite. Once again, it depends on what you are trying to prevent (the underlying condition that you are trying to prevent).
4. In case of infective causes, treatment with suitable antibiotics would help in resolution of the splenic enlargement. However, for other conditions, the treatment would be different.
