Suggest Treatment For Sharp Abdominal Pain And Constipation

Question: Pain in mid to left lower abdomen with occ. sharp pains into my perineal area for one day, then lessened yesterday and almost gone this morning. I took Dicetel when this started. I also have haemmorhoids and IBM. I really strained to have a BM a half hour ago after taking Metamucil last night. The stool was very large. When flushing the toilet I noticed that one of 2 stools had bloody mucous in it....bright red to maroon in colour...also the colour of the juice I have been drinking regularly for the past few days. I also am just over a cough...up until yesterday was swallowing lots of mucous from my sinuses. Maybe the pain was from a stained muscle from coughing and also because I was moving heavy lawn furniture into a shed yesterday. I am leaving for the States in two weeks and don't want to see my doctor as this causes implications for travel insurance. I will be away for the winter staying with my partner. If I see a doctor, I won't be able to go because I won't be able to afford insurance with a pre existing condition. That would make me so very depressed as I have done so much to get ready and want to see him very badly. I know you will have to cover yourself so I expect you will tell me to see my doctor. This could be very difficult anyways as it's Friday and this is our Thanksgiving holiday weekend here in Canada. I guess I would like you to tell me not to worry and check the next BM. It there is no blood, maybe it was a one off. I know my hemorrohoids are bothering me. Otherwise I feel great. Thank-you.
I am sure the blood is from a hemorrhoid. I just placed tp at the edge of my rectum where it is sore and there were spots of bright red blood on the tp.

Detailed Answer:
Hi and thank you for asking!
Wish you Happy Thanksgiving!

I read carefully all your concerns and I understood your query.

You are reporting to have sharp abdominal pain, constipation.
Your symptoms seems to be closely related with constipation and bowel peristaltic.

The fact that the symptoms has disappeared after BM and stool softener indicate that all your abdominal pan is related with constipation.

With regards to blood in stools it can be caused from hemorrhoids and constipation.

The fact that the blood was red and fresh shows that it is from the lower part of rectum or anal sphincter. You have a history of hemorrhoids , so your symptoms are well explained.

If you were my patient I would recommend as follows:
- use plenty of water
- use vegetables and fresh fruits
- use olive oil 3 teaspoon per day
- increase physical activity
- use fiber packs 2-3 per day
- use preparation H(OTC) for anal area
- keep a good anal hygiene
- use Metamucil when you are constipated

If the symptoms will aggravate than you should go to your physician.

Wish you fast recovery

Thank-you Dr. Klerida. I will follow your recommendations. My BM today was perfect!!
One question. What is the difference between a fiber pack and Metamucil? What would be the brand name of the pack because I see that there are Metamucil packs. I guess I am confused when you say just to use Metamucil when constipated and the packs regularly. Thank-you for clarifying this. Oh, I got a response from a gastric surgeon. I didn't know I would get several responses. I like your approach but he said I should get have a proctoscopic exam done to rule out rectal cancer at some point. And a colonoscopy... I feel this is all due to a little tear or hemmorhoids and intend to follow your recommendations for now...unless symptoms aggravate as you said. Thanks again.

Detailed Answer:
Hi back my dear

There is no difference between fiber packs and Metamucil.
The difference is that Metamucil is a brand name.

I review my answer and I saw that I have mentioned twice fiber packs. But it is the same thing Metamucil and fiber packs. Sorry for that.

You can use enema and stool softeners or laxatives when you are constipated more than a week.

if your symptoms will aggravate than please follow the suggestions given by your doctor.
Wish you all the best!
