Brief Answer:
Thyroid dysfunction.

Detailed Answer:
Hi,
Thanks for posting query to HCM.

Anti thyroid peroxidase antibody(Anti TPO)>1300 indicates your thyroid dysfunction.
It is known as an Autoimmune thyroiditis (Hashimoto's thyroiditis )

Have you done TSH,FREE T3 and T4 tests?

Shoulder pain could be related to thyroid.
It can cause muscle fatigue or
Your thyroid gland might have enlarged in size.
It can compress the nerve which leads to shoulder and neck pain.

Since it can causes lower thyroid hormone levels
I would suggest you to visit an Endocrinologist
For clinical examination
For further investigations.
To switch over to thyroid medication.

Meanwhile I would suggest you take painkiller Ibuprofen for shoulder pain.

Hope I have answered your question.
If you have any questions I will be happy to help you.
Thank you.
