Question: I have severe pain in left buttock. but am told the pain is coming back.

I would need a wider clinical picture and more descriptions to hone in on exactly what may be causing your pain, however I will give you a few ideas. If the pain is as the doctor said, coming from your back, than it is likely from nerve impingement. A nerve which exits your back (more specifically lumbar region) if impinged, or pressure is placed upon it, the result will be pain to the distribution of where that nerve normally sends and receives signals. In this instance it may be the superior gluteal nerve, it's branches and or local nerve fibers. Are there any other symptoms other than pain you are experiencing? Other possibilities, other than what your doctor has described are piriformis and or local muscle spasms or irritation.
Treatment for your symptoms can vary, but to alleviate the symptoms and attempt on improving your clinical picture I suggest muscle stretching when possible, a mix of heat and ice application to the region, muscle relaxants, anti-inflammatories and if possible a schedule with a physician to obtain imaging to assess where exactly the pain is coming from. Also of use may be steroid injections, depending on what this is.
