At your advanced age there are many causes of the severe pain in your leg and ankle and need to be ruled out before I could advise on your treatment.
Which anti-coagulant you take?
Update the last bleeding time test done with advise of your doctor,if any?
Any tobacco use -smoking / or chewing?Any alchohol ? and its frequency and since how long?
Last medical check up you did with its reports,if any?
Does the pain start after the stressful walking ? or after a long resting event?

Hi,Dear thanks for the reply.Sorry for the late reply,as I was busy with one critical case.
I reviewed your query meticulously, along with the update reply from You.
I understood your health concerns,about extreme leg and ankle pains and the risk of the recurring leg clot.
As I am without much of the information of your health status,I tried to understand it, by My last reply.

With whatever information and facts are given by your query and your update replies, My answer is as follows-

Scientifically YES,There is a definite risk of recurrence of the Clot,even though you might be already taking blood thinners for the clot,which you had 20 yrs ago.

This risk is due to following reasons ..

INR(Internaitonal Normalised Ratio) may get disturbed due to many variables like diet with high Vit K intake/drug interactions/Estrogen intake for other illnesses/
Reduction of physical activity/compromised hear functions/repeated infection chances,which are common interfering factors at this OLD-age.

Your body machine with its inherent clotting defects,is the same and would get worst day by day due to aging.So by aging the coagulating functions get more compromised due to reduced functions of the Liver.

My opinion in your case-
No Reason to Worry in your Case.

In your case, as the extreme right Leg pain is confirmed to be due to Heel Spur,You should not worry,as that pain is not due to another or /recurring blood clot in your leg,as feared from YOU.
So DON'T worry at all.
I would advise you NOT to have any anxiety as the extreme leg pain is confirmed to be due to Heel spur.

Remedy for the Heel Spur-
Consult ER Ortho-Surgeon,who would treat it on the following lines
Rest
Hot Fomentation locally to Right Heel
Tab Anti-inflammatory drugs /or Locally apply Gel(as Voltaren)
Do quadricep and Calf exercises 4 times a day, while in the bed.
Be Ambulatory as far as possible.
Check with INR ,while on anticoagulant once in 3 mths time
Take anticoagulant doses under cover of your doctor.
