Question: I have severe arm ache which comes from the back of my left shoulder blade and runs down the back of my arm in to my elbow can you please advice

Hello. Thank you for your question and welcome to Health Care Magic. I understand your concern.

The description of your pain mostly fits the symptoms of an acute musculo-skeletal soreness, which may come due to assuming inappropriate position of the area for a certain amount of time, such as during sleep.

Now, the area of your pain probably involves the deltoid muscle (shoulder muscle) which is a muscle fixated both on the shoulder bone and on the arm bone and/or the trapezius muscle (the neck muscle.

I would recommend you to try a course of 5 days with an non-steroidal antiinflammatory drug (NSAID, such as ketoprofene or ibuprofen), accompanied with stomach innermost layer (mucosa) protector (Ranitidine, Pantoprazole). The pain will probably not go away, immediately, therefore, my opinion is that it may last for 1-2 days.

Although the above said is the mos probable diagnosis, according to your age of 63 years old, I would recommend you an additional test, just to be "in the clear": a 12lead-EKG (to exclude any possibility of something acute going on in your heart muscle, since ischemic heart disease sometimes can be heralded by atypical signs).
