Detailed Answer:
Hello and welcome,

Anytime there is extreme pain, and it is a change from how it has been, it is best to go in to be seen.

I would start first by calling your doctor's number. You will likely be given the doctor on-call for your doctor. Explain what your doctor has told you so far, and that the pain has increased now. The doctor on-call will either phone you in stronger pain relieving medicine, or have you go in to the ER. Either way, they may just give you enough pain medicine to get you by for a few days until you can get back to your primary doctor. Probably only a few days worth because they worry about their DEA license (rules are getting stricter for doctors). When your primary doctor returns to the office, call and explain when you will be seen by the hand specialist (hopefully you'll have an appointment scheduled by then) and that you need help until you are able to see the hand specialist.

I hope this information about navigating the system helps. Please let me know if I can provide further information or clarification.

Yes, go to ER.

Detailed Answer:
If your doctor has no mechanism for contacting him or her during non-office hours, then yes, go to the ER.
