Posted on Fri, 10 Jun 2016
Question: I have been having a lot of migraines. I had terrible ones when I was young. I did not have any for over 25 years. They started about 10 years ago, but were not bad. I had a bad one yesterday and also numbness in my left arm. Hoping it is just a migraine?

Brief Answer:
Diagnosis and treatment of Migraine

Detailed Answer:
Hi XXXXXXX thanks for asking from HCM.

I can understand your concern. Migraine can recur after long time. Recurrence happens due to number of factors like

: Emotional Stress
: Sudden change in season
: Sleep cycle change
: Recent use of alcohol or smoking

If there is any factor which is positive in your case, it could be the reason. Confirmation of migraine can be done by evaluation of signs and symptoms like

: Headache involving half of head
: Fear from light, watering in eyes, redness, blurred vision
: Before headache, presence of aura like abnormal smell, abnormal taste, nausea
: Headache with vomiting
: During headache, there can be pain, tingling and numbness like sensation in half of body.

You can correlate your features and confirm about "Migraine". If still in doubt, consult doctor about it.

Regarding treatment of migraine, it can be treated after confirmation. You can follow

: Preventive therapy - It includes treatment which is aimed to delay attacks of migraine.
; Avoid trigger factors like stress, change in sleep cycle, bright light, food like cheese, chocolate, sweet food, physical stress.
; You can take medication like
beta blockers like Propranolol
antidepressants like Amitriptyline, Nortriptyline
anti epileptics like Gabapentine
You can consult doctor about medication and start it.

: Abortive therapy - It includes treatment which is aimed to treat pain attack.
; Painkiller like NSAIDs e.g. Diclofenac sodium, Ibuprofen.
; Triptans like Sumatriptan, Almotriptan, Eletriptan etc.
; Acetaminophen
; Ergotamine

Along with painkiller, antiemetics are given to treat vomiting like
; Domperidone
; Domperidone with Pantoprazole combination

Consult doctor about abortive therapy. Usually NSAIDs are prescribed first and if no relief, then stronger painkiller.
Before starting medication, get cardiac/respiratory/renal and liver function evaluation as drugs have multiple side effects.

Hope it will help you. Still in doubt, kindly let me know.
Thanks. Take care.
