Thanks for your kind reply. I am giving the details of my illness
1. This attack (lower back pain) is since 20-25 days. But similar episodes have been happening since the last 20-25 years. The specifics are:
The lower waist pain is localized at a certain region which sort of gets jammed when i tend to bend say to lift something or to wear shoes it gives me jerk/spasm which is unbearable. It is similar to a hinge of a door which would require lubrication for movement when it gets jammed. I visualize that adding oil may remove this jam. Besides this, the stiffness is also in elbows, finger joints, knee but comparatively of negligible intensity, but yes it is there.

Sir, I am a fitness freak to the extent of addiction. I exercise daily - aerobics, stretching and bending. I also do abdominal crunches. But this problem has become sort of chronic and is affecting my profession and quality of life. When i don't exercise, my energy level goes down. Right now I have been advised complete bed rest and following medicines:
Texxa 150, Acenac MR, Pregamet 75, Pantocid D.
If you require any other information i shall be glad to furnish the same. May i add that i also have this cervical spondylitis at C3 and C4 which i keep in control by exercising. This has also resulted in almost fatal vertigo in the year 2001.

Hello,
It is good that you are concern about your fitness and doing exercises daily. It is also a good news that there is no leg pain or weakness as it suggest there is no nerve compression in the back.
I have studied your MRI and there is one cyst which is causing bone destruction and that may be the cause of your back pain.
I would suggest you to show it to a neurosurgeon. He might advice you a CT scan guided biopsy or a surgery to remove this cyst. As if this cyst remain there then chances of nerve compression and weakness may be there.
I would also suggest you to avoid weight lifting and doing any such activity which can put sudden heavy load on the spine as it can cause collapse of the spinal column.

Thanks for your kind advice. I am uploading the advise of both neurosurgeon for your kind perusal. Both have ruled out any serious concern pertaining to the cyst. I have started the treatment (Dr. Jha's) from today. Please advice.

It is good that your neurosurgeon found that this cyst is not harmful though I feel that this is the cause of your back pain. I would suggest you to show it to some higher centers if possible in XXXXXXX It is not common to have a cyst causing compression and destruction of the bone. I do not think that it can be ignored.

Thanks for you kind suggestion. Is there any way of showing the MRI films (5 Nos.) to you. Can you suggest the name of higher centres in XXXXXXX / Hyderabad? What would be the treatment of permanent remedy of the cyst and the approximate cost involved? Can I come to you for treatment?

Hello sir,
There is no need to send those MRI to me as I have already studied your reports. I think you need to show it to a spine surgeon. I have few known doctors in XXXXXXX One of them is Dr. XXXXXXX Bapat in Kokilanben Dhirubhai Ambani Hospital,Andheri West.
You can tell him my name and he will guide you properly.
I think at least we should get biopsy of that cyst. Cost is not too much for it, But you need to ask to doctor who is doing it.
I do not operate such cases at my center. But if you want you can share any other information with me if you want to know any other thing.
In last I want to say that may be this cyst is not dangerous, still I think it is need further investigation.

Thanks for your concern regarding the cyst and related advice. Right now i am taking complete bed rest. As mentioned earlier my problem is chronic. The stiffness is in whole of the body joints in general. I must clear specifically that i am not suffering from any kind of PAIN. It is only stiffness, spams, jerks which occur while bending down and the lower back area gets sort of jammed. My uric acid is within the normal range, i don't have arthritis either. Can you suggest some higher center in Hyderabad? Being on medication i am feeling very nice but not sure what happens after treatment is stopped.

I have studied your case and I understand that you are having stiffness of the back since long time. I also understand that cyst was found because you got x ray and CT scan. But now our first priority is to find out what type of cyst is this? I do not aware of any doctor from XXXXXXX but you can show in Nizam Institute.
For stiffness I have few suggestion which you may have tried in past,I would recommend you to continue with them.
1. Hot water fomentaion
2. Muscle relaxant like Myoril or Myospaz
3. Tablet Voveron whenever there is also any pain.
4. Avoid sleeping in very cold temperature.
5. Back muscle stretching exercises.

I do not think think that this test was needed in your case. But this is normal in you. So do not need to be worried about this aspect. HLA b 27 is positive in young person in back pain and it is suggestive of ankylosing spondylitis.
