Question: Hi Doctor. I am a 25 year old male. I have suffered from depression since my adolescense. I have trouble organizing things, planning, prioratizing and being motivated which varies in degree at different times. There has been times when I lacked energy even to get out of the bed. I feel aimless and emotionless most of the times. Finishing tasks is like climbing a mountain for me. I can't decide where and how to start. I get very less or no satisfaction even when I accompalish big things in life. I have done fairly well so far but think of myself as an underacheiver. Managing relationship has also been a problem for me. I am unable to live my life to the fullest. Are these signs of ADD. If yes what can I do to manage my symptoms?

Detailed Answer:
Hello XXXXXXX

Thanks for writing to us. I can understand your concern.

Many patients who have diagnosed depression earlier later on found suffering from ADD (attention deficit disorder). In XXXXXXX it is under diagnosed condition in adolescents and adults.

Attention Deficit Disorder (ADD) of adult is a psychiatric condition in which person has difficulty in concentrating at a particular task, restlessness, and irritability and secondary occupational/study or social disturbances. Many times it is there with depression also. History of similar symptoms in childhood should also be evaluated to reach diagnosis. Diagnosis can be confirmed through detail history and mental status examination which should be done by a psychiatrist/specialist.

In your case from available history it is possible that you may have ADD with or without depression. Any personality problem should also be ruled out.

Physical symptom like lack of energy with other psychological symptoms is there. In that case I recommend going for thyroid and vitamin B12 level also.

After confirming diagnosis of ADD drug from amaphatamine group like Methyphenidate will be helpful. Simultaneity cognitive behavior therapy, supportive therapy, relaxation techniques and social skill training should be done to improve overall quality of life.

Hope I have answered your query, I will be happy to help further.
Regards,
Dr.Chintan Solanki.
