Question: I had sever cramping, vomiting and diarrhea starting Tues. 1 am. Bloody diarrhea continues today. No temperature as far as I know. I feel kind of ok but the continued bloody diarrhea is concerning. I'm eating a bit but not too much meat or raw vegetables. Fairly low energy. Stomach still tender to the touch.

77 year old active, athletic male, good health otherwise. What should I do? Thanks.

Amoebic gastroenteritis.

Detailed Answer:

Hi,

Most likely you are suffering from mild gastroenteritis, usually gastroenteritis is by viral infection(Nova virus) and is self limiting, it takes five to seven days. But as you mentioned that you are having bloody diarrhea you could be having either bacterial or protozoal infection or a mixed infection. Usually protozoal / Amoebic infection is associated with bloody stools.

I would suggest you to go for a few tests like ultrasound of the abdomen, stool test under microscope, complete blood count and total and differential leucocyte count.

If stool test is positive for bacterial or protozoal infection. Start taking Zithromax (Azithromycin 500 mg once daily) and Secnidazole 1 gram daily for 5 days.
Along with it take Fybrogel husk. It will increase the bulk of your stools.

As of now take:
- Tab Imodium twice daily to control frequency of motions
- Tab Zofran 4mg can be taken SOS for nausea and vomiting (Both are over the counter)
- Eat light and easily digestible food
- Drink plenty of water and electrolytes solution Electral and ORS to maintain energy levels and hydration
- Monitor pulse and blood pressure twice daily
You would be fine soon.
