Question: Severe cramping of the hands. 49 years old. I have had dupretens (spl) surgery on one hand. It just started today.

Enzymes i.e collagenase clostridium histolyticum have been recently approved by FDA for treating dupuytren contracture. In case of flares associated with joint swelling and tenderness, corticosteroids can be resorted to in a tapering course over 3 to 4 weeks. NSAIDs like nimesulide or celecoxib can be used in the meanwhile. Fasting blood sugar and HBA1C levels should be done to rule out the possibility of neuropathic pain associated with diabetes. A rheumatologist should be consulted to examine the hand and wrist to look for possibilities like carpal tunnel syndrome or rheumatoid arthritis.
