Brief Answer:
A comrehensive follow up is mandatory for an optimal strategy.

Detailed Answer:
Hello! Thank you for asking on HCM! I understand your concern. Severe calcification of aorta on that regions needs a comprehensive investigation of renal function, as well as detection of possible lower limbs ischemia. As long as renal function is OK, no deterioration is detected, and no clinical sighs of limb ischemia is present, we just have to observe the patient and be careful for any possible complications. A lower limb vessels Doppler ultrasound is mandatory, as well as angio CT (or angio MRI) to appropriately follow up the patients. If ischemia is detected, then surgery is mandatory (or stenting if feasible). You need to be in close relationships with a vascular surgeons to build an optimal scheduled follow up.
I recommend to control high blood pressure with an appropriate anti hypertensive therapy, to treat dyslipidemia (with statins), and a relatively active daily profile.
Follow up is of atmost importance to determine the strategy in advance.

Hope to have been helpful to you.
