Suggest Treatment For Severe Anal Pain In A Child

Question: Hello!
I have a 7 year old daughter who recently started to have severe anal pain, only around 11 at night. She has no problem with her BM's, (it started on a visit to her father's in another province, but has not healed since she's been home) and has no blood present, no rash, and in the day she says it bothers her but not too much. She runs, plays, etc, like all normal children. When it hurts, it throbs or stabs (her words, it's pokey) and I give her warm baths with baking soda. Tonight I gave Tylenol jr. and will try to find a walk-in clinic tomorrow. What could it be? I thought it might be haemorrhoids, fissures, but I don't know much about this type of problem and want my daughter to heal asap! She has also said it feels like she has an incomplete BM, there is pressure. She drinks lots of water, eats lots of fruits and vegs and isn't a kid who goes crazy for sugar or processed foods, I'm really at a loss as to what else to do for her. Please advise!

Based on the history you have shared, in the kids of this age group, most common cause of having peri anal discomfort or itching is worms infestations. It's better to deworm kids with these symptoms every now and then with Oral antihelmenthic drugs in one or two doses, after discussing with your GP. However local causes like you have mentioned also needs to be ruled out in rectal examination.

For comfortable bowel habbit, as you have mentioned that she complains about not having defecated completely, stool softeners like lactulose or local glycerine suppositories can be used and these symptoms will get relieved. Kids usually have soft intestinal mechanisms so antispasmodic like drotaverine are usually avoided with such sensations due to potential side effects.

It's better to have stool analysis done and continue the other measures like foods precautions and OTC analgesics like acetaminophen till then.
