Suggest Treatment For Severe Acid Reflux And Hemorrhoid

Question: My wife aged 53, is having hyper-acidity problem (acid reflux, heart burn). The problem aggravates if high protein food is taken. This affect digestion of food also. With medication and control of food she manages the condition and it is under control. She had removal of ovaries and uterus in 2002, due to endometriosis.
She is also suffers from external piles. whenever the swollen is observed (may be once in a year or more), she takes Pilex for around 10 days and gets cured. Whenever she takes Pilex, her acidity, heart-burn related problems get aggravated.
Recently, the swollen condition re-appeared, but she did not start taking Pilex and the swollen condition got increased and she had bleeding after passing stool, for the first time. She started taking Pilex one tablet, twice a day. She already took 60 tablets (one bottle), for 30 days. Bleeding never occurred again, The swollen condition reduced, but did not cure completely as before. She have soft stool, and she do not have problem in passing stool and she generally have 2-3 motions in the morning. However, after having 2nd or 3rd motion, the swollen part comes out of the rectum, and requires to be pushed inside. She also applies Pilex ointment, after warm compression in the affected area.
Please tell me, how long can she take Pilex tablet?
Should she try other treatment?

She also suffer from gas formation, bloated belly and her bowels seems to not clear completely. Her stool is soft but thinner in shape.

I have gone very carefully through your query.

​​Your wife's problem seems to be due to poor peristalsis. ​​Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation. This is also responsible for her constipation and therefore getting piles.

​​​​In order to relieve yourself, she needs to do the following:
​​* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Take omeprazole once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fibre intake.
* Regular morning walk and exercise to keep the bowel movements proper.* * Pilex can be stopped and need not be continued further.
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.

Dear Doctor,

Thank you for your kind advice Sir.

The following I want to add:

The type of diet you have mentioned is mostly being followed by her, however, she cannot take curd as she noticed that it increases her acidity.

As already mentioned by me that, " Her stool is soft but thinner in shape", but occasionally she have a feeling that her bowels have not cleared completely and during that time her acidity and gas problems aggravate.

Even though she have soft stool, can this condition be rerered to as having constipation?

No issues if she cannot take curd. But it is very essential to increase her fiber intake. Her not getting a sense of clear bowels is a form of constipation and can be attributed to sluggish bowel movements and inadequate roughage in diet. She needs to take plenty of green leafy vegetables with diet and possibly fiber supplements like psyllium husk or senna as well.
