Brief Answer:
Medication, plain Sinus CT.

Detailed Answer:
Hi,

Thank you for your query.

1. Symptoms of sinusitis lasting more than 6-8 weeks are considered as Chronic Rhinosinusitis (CRS). This may also include Allergic Rhinitis (Frequent colds, running nose) and asthma like symptoms (lower airway symptoms).

2. Ideally a nasal examination and nasal endoscopy helps rule out any physical obstruction such as septal deviations (DNS), turbinate hypertrophy, septal spurs, nasal discharge and so on. The appearance and colour of the inside lining (mucosa) of the nose also gives clues to the exact diagnosis.

3. First a two week trial with medication is given. This should include an anti-histamine (such as cetirizine, fexofenadine) , an anti-leukotriene (such as montelukast), a steroid nasal spray (such as mometasone, fluticasone) and regular steam inhalation. You will require a prescription from a local doctor for these medications.

4. A plain CT PNS (Para Nasal Sinuses) is the gold standard investigation for Sinus issues. If it is normal, you do not have sinusitis. If it does show any sinusitis, further treatment can be discussed if you upload the CT images.

5. It is important to remember that allergy has no cure. However, it may be controlled by the above medication. Long term resolution also requires treating any unhealthy sinuses.

6. Kindly follow up after two weeks of medication and the Sinus CT images.

If you have any more questions I will be available to answer them.

Regards.
