In this case with atrial fibrillation and enlarged righ atrium the best strategy is rate control and anticoagulation (blood thinners).

With rate control we understood that your HR be <85/min at rest and <120/min at activity. To reach the goal i recommend the use of betablockers and digoxin (if betablockers alone are unable to do so).

Anticoagulation is necessary to avoid pulmonary embolism and stroke. The I.N.R should be kept between 2-3 in order to do so. I recommend the use of Qoumadin (Warfarin).

In the presence of other symptoms (than tachyarhythmia) specific treatment may be added. For example, in the presence of dyspnea diuretics may be added. If hypertension than Ace-i or ARB may be added.
