Suggest Treatment For Red Dots On Hand And Back

Question: My brother has got small red dots on his right hand and also on his back. Please suggest what could be the reason for this.

It is been more than 2 weeks it started showing up but he has not consulted the doctor yet.

Please check the attachments to see the red dots.

I would like to know if he has any other symptoms associated with these like itching?

The spots on back seem like acne to me. Did he previously also develop these lesions on his back Or this is the first time?

I would like to know if there are any other sites involved like abdomen, thighs, genitals etc?

Does he have fever?

Answer : No itching. He developed this condition 4 weeks ago.. the lesions stays for a week and turns black and the spot gets corrected by its own over time. New rashes in the group of 4-5 gets formed again and the cycle repeats.

The spots on back seem like acne to me. Did he previously also develop these lesions on his back Or this is the first time?
Answer : He had these rashes due to dandruff fall but were confined near shoulder area. From last few weeks the problem got widespread and extended to back as well..

I would like to know if there are any other sites involved like abdomen, thighs, genitals etc?
Answer : skin in other areas is healthy. This is only on right hand and back

Does he have fever?
Answer : No fever. Last time he had fever was over 6 months ago..

Acne; Oral and topical antibacterial antibiotics

Detailed Answer:
Hello. Thank you for details.

I will keep a possibility of acne vulgaris which can also affect the torso, shoulder and arms in addition to face, the usual site.
If I was the treating doctor I would suggest that he be started on Oral antiacne antibiotics e.g Doxycycline capsules 100 mg twice daily for 2-4 weeks.
Topically he may use an antiacne antibacterial e.g benzoyl peroxide 4% cream (Brevoxyl cream), twice daily, over the whole affected area.
Dark spots left over by resolving lesions would take time to fade but benzoly peroxide is also a bleaching agent and would therefore help fade away the marks as well.
For dandruff he may use either a Ketoconazole based shampoo e.g Nizral 2% shampoo every alternate days Or he may use a Piroctone Olamine based shampoo e.g Sebamed antidandruff shampoo, every alternate days.
These are prescription drugs and should be used under supervision of a doctor and therefore I suggest that he visits a dermatologist in vicinity for the needful.

Hi,

We visited a Dermatologist today and he suggested for a scalp and back fungal infection test(total 2 tests). Since the lab was closed, we thought of doing it tomorrow.

Do we really need to do this test as it seems to be very expensive. Doctor has not suggested any medicine and told he will give it after seeing the report.

Hi. I would like to know which all tests?

He must have kept a differential diagnosis of pityrosporum folliculitis which is a fungal infection caused by a yeast malassezia furfur. Pityrosporum folliculitis can present similar to acne on torso, shoulders, chest etc but usually it is very itchy. However you said that there is no itching at all therefore i already ruled it out. Moreover I don' t think there is a need to test for fungus from scalp because it would anyways be positive if your brother suffers from dandruff. However the tests that a doctor orders is entirely his or her own discretion and depends on his or her understanding of problem.
