Detailed Answer:
Hello!

Welcome back on HCM!

I understand your concern and would explain that in this situation it is necessary performing a quantitative measurement of PVCs, by performing an ambulatory 24-48 hours ECG monitoring.

If in the Holter the PVCs result to be markedly reduced (by 80%), it means that the drug with the given dose should be considered effective.

Otherwise, an increase in the dose may be necessary.

From the other hand, you should consider also other possible triggering factors of PVCs:

- increased anxiety
- caffeine intake
- electrolyte imbalances
- possible thyroid dysfunction.

That I why I would recommend performing some blood lab tests (complete blood count for anemia, thyroid hormone levels for thyroid dysfunction, blood electrolytes).

I would also recommend reducing caffeine and alcohol intake.

Anxiety could also play an important role in this clinical scenario. You should consult also with a specialist of this field (if all the above tests result normal), in order to help you manage better anxiety.
Psychotherapy, Yoga and recreational sports can also be useful in helping you with anxiety and also PVCs.

You should discuss with your doctor on the above issues.

If you have any other questions, please feel free to ask me.

Kind regards,

Dr. Iliri

I believe I know the issue. I have a peripheral neuropathy that has long activated my sympathetic nervous system. I fell on my wrist in a bicycle accident last august. it still hurts on a daily basis. I think I have nerve damage there that has given me nerve fascination in my left pectoral as well as above my left eyebrow. it has rubbed my nerves so raw that I have been experiencing sympathetic burnout on some level. just this knowledge tonight has made them go away. amazing.

Detailed Answer:
Hello again!

Thank you for the additional information.

As agree with your that this may have some implications in your clinical symptoms. As I explained physical an emotional stress can activate sympathetic nervous system, causing PVCs.

From the other hand, anxiety seems to play an important role in all this clinical scenario. The fact that the knowledge made your symptoms disappear, indicates that most of your symptoms are related to anxiety.

So, I would recommend discussing with a specialist of this field, in order to help you manage better your anxiety. An antidepressant would help you, keep your mind away from PVCs and try to think about more beautiful things in your daily life. It can help you achieve a new relaxation level.

Yoga, meditation and recreations sports can also be very helpful in this situation. You should know that physical activity is the best doctor for a healthy body and mind.

You should try to enjoy your life, because there is nothing wrong with you! Just relax!

Hope to have been helpful!

Best wishes,

Dr. Iliri
