Suggest Treatment For Psychosomatic Disorder

Question: I had an allergic reaction to diet tea the summer of 2013 and went into depression. I was already on an antidepressant and had to switch to Triavil. Triavil worked great for about 4 months. Then last Christmas Eve I woke up feeling terrible and worrying. I have tried switching to several antidepressants this year but could not stand the side effects such as anxiety, panic attacks, jittery, etc. I tried Vibryd for less than a week in April. I did not stay on it but a few days because it made my low back hurt so much. Then in May I tried Britellix. My back was killing me on this med. so I quit taking it. I only stayed on it maybe 7-10 days at the most. Off and on my back would hurt or get stiff after taking this drug. This lasted for probably 9-10 weeks. The pain was not bad, but my low back would hurt every once in a while. Not every day. Then in mid July I tried Paxil. Immediate pain in my stomach, nausea, etc., low back pain. Then I started having pain going all over my body. The aches and pains went away after 2-3 weeks except the stomach trouble and the low back pain. The pain was so bad in my low back I finally got off of the drug and went to Celexa. I soon as I got off of the Paxil the back pain went away. At first the Celexa did ok, then after about a week I started having back back pain on the Celexa. Also, aches and pains in my body. Mainly my arms and hands. My stomach was still bothering me. I could not stand the pain anymore so I went back to Triavil. This time the Triavil made me have low back pain and pain all over my body. I could hardly walk up and down stairs. I finally got off of the Triavil. I still have stomach problems caused by the Paxil. Nonstop indigestion and bloating. Sometimes stomach cramps. I am still having some aches and pains in my body but not as bad as being on the last few antidepressants. All I am on is 2 pills of Tranxene a day. 3.75 mgs. I don't feel depressed, but I am not motivated and I am still having pain in my stomach or ache in parts of my body. I feel so bad and can't get my mind off of the pain. With all of the switching I have done this year with meds. I was gradually trying to cut down on the Triavil. I was still on some Triavil every time I was trying a new med. except when I went to Paxil this summer and Celexa this fall. I wish I had stayed on 1 1/2 Triavil. I was not having any stomach trouble or aches and pains all over my body. So now I have aches and pains all over my body, stomach hurting/indigestion, etc. , now insomnia. My body is screwed up big time. Do you think all of this switching meds caused fibromyalgia? I can tell the nerves in my head are not healed. I don't know what to do. The doctor I go to want me to try Abilify. I should have never tried the last two meds. I would not be where I am today with my health.

Brief Answer:
You can be helped with duloxetine and flupirtine

Detailed Answer:
Hi XXXXXXX

Thanks for writing to us.

You have mentioned lot of details of switching and I read it carefully. I understand your concern.

Points to be noted:

-Most of the medicine for psychosomatic disorder should be taken for long (to say 3-4 weeks) to see its effect under strict medical supervision.

-If you cannot tolerate with intolerable side effects, you can stop after discussing with doctor. Reintroduction of the medicine done only when no other alternative.

Main concern to be treated is your back pain. For that, first of all any spinal or nerve problem must be ruled out with help of orthopedic doctor. If it is confirmed that there is no physical factor, probable diagnosis is somatic pain/fibromylalgia.

For that in my opinion Duloxetine and flupirtine maleate are very good medicines with least side effects. I get results in almost 90% of the patients with these medicines in such type of problem.
Simultaneous supplementation of Vitamin B12 and Vitamin D as well as physiotherapy would also help.

Discuss my opinion with your doctor and move forward for further treatment.

Hope I have answered your query. I will be happy to answer more if still other queries.

Regards,
Dr.Chintan Solanki.
