Suggest Treatment For Pimples And Its Marks

Question: Hi Doctor. I am facing pimples problem & the pimples r leaving dark marks. See the images that I have uploaded.

I was having a fair complexion & due to my marketing job (I keep travelling daily), my skin became dark. I visited a Dermatologist 6-7 months ago & he prescribed (A) Kojivit gel & (B) Fluticasone Cream I.P - Flutivate skin cream. My skin regained the complexion but from then I started facing pimple problems.

Two months ago he prescribed when I visited him, he prescribed (1) Clindamycin Phosphate Gel USP (1%), (2) Doxycycline and Lactio Acid Bacillus Capsules, (3)
Adapalene Gel (0.01%). He had told me to visit after two weeks. There was no improvement.

When I saw him after two weeks, he again prescribed (1) Facelin Gel (To apply in morning), (2) Benzac AC 2.5% Gel (To apply in night), (3) Ahaglow S Facewash & (4) Cynomycin 100 mg Caps (Morning & Night for 4 days).


I am using this from almost two weeks now but still no improvement. Though I am applying these creams regularly, I am getting new pimples & they are leaving marks. More on forehead. Also on cheeks but not worse like forehead.


Please help. Pls give some effective medicine so that the marks will be removed & to stop pimples. I know that u cannot prescribe online but pls list out the medicines with clear instructions so that I will visit a doctor, show your reply on my mob /laptop & request him to write the same medicines.

Forehead is looking very bad with lots of marks doctor. Being a girl its not very easy to have these marks. Pls help

I understand your concerns especially after having been in the profession for nearly three decades. There is a lot that can be done. It needs good planning and careful implementation. I will try to do my best despite the constraints of Telemedicine.

I would have advised following to my clinic patients:
1 Clindamycin gel locally thrice a day
2 For the time being,please continue Cynomycin but 100 mg once a day only
3 Cap Vizylac once daily
4 Tab Antoxid HC once a day
5 Minimize sun exposure/use sunscreen, Other preparations may be kept on hold for now

Please get your Liver Function Test and Lipid Profile done as you might be needing Retinoid therapy to help stop new eruptions of pimples. This treatment is suited for those who are not going to conceive in near future and is always given under supervision. The best part is that it is much superior to antibiotic therapy.

I think you can make good choices about your requirements and seek specialist help if needed.

Thank you for replying.

I understand that I have to take this medication:

1 Clindamycin gel locally thrice a day
2 Cynomycin 100 mg once a day
3 Cap Vizylac once daily
4 Tab Antoxid HC once a day

Pls correct me if I am wrong. Also, how long should I continue this? I keep getting different medicines /creams each time from the doctors but condition has not improved. So will this medicines work? I hope you can understand my problem, how am feeling right now.

You are correct in stating about the medicines that have the best potential to help you.

It would be good to ensure that you area good candidate for Retinoid treatment. I am sure that will work to great advantage as that has helped each and every patient of mine till date.

Please keep on washing face 6-8 times a day with plain water as it helps to unclog the pores and improves skin health.It is also important for you to understand that treatment takes several weeks to show up results and the initial phase may not be that good as some individuals get flare up lasting one or two weeks but eventually most patients settle nicely.

Once I am sure that Retinoids will be safe for you, I would like to suggest you on those lines.

Sorry, I wasn't clear. Should I take these medicines or should I consult a Dermatologist for Retinoid treatment!?

or u mean I have to continue the medication suggested by you for two weeks & then contact you back to ask about Retinoid Treatment?? so that u can guide me whether I have to undergo Retinoid Treatment. right?

In the meantime, tests can be manged and further treatment can be carried out with the supervision and counseling by a doctor. I will surely follow your advice & continue with the medication. In the meantime, I will try to get those tests done..

My other doubt is - "Clindamycin gel" which I am supposed to apply thrice a day : How do I manage to apply thrice? because I come to office in the morning. I can apply in the morning when I wake up, keep it for few hours till I get ready & wash it off while taking bath & I can apply this while sleeping in night & wash off in the morning. But how to apply three times (Can I just carry it to office, just apply in the noon like how I apply face cream & keep it till evening)? This gel would be oily so I wont be that comfortable in office. What do u suggest doc?


Also, as it is winter, I also should apply moisturizers /vasellines to my face in the night when I go to sleep.. How to manage those as I will be applying this gel in the night!? also, by using this gel now in winter, wont my skin loose its quality & become rough /dry

I have read your concerns and have also come up with the solutions.

1 Clindamycin is not oily. Hence can be applied anytime. One may use it before going to work, after return from there, and then at bedtime thus completing thrice a day schedule. During day time several face washes and a moisturizer called Efaderm, easily available, can take care of dryness. Vaseline is not recommended.

2 Retinoids use needs medical supervision and other concerns too which can be best addressed by face to face discussion with dermatologist. I have sought you to undergo investigations so that precious time is not wasted and when you visit dermatologist, you already have the reports with you and can be put on treatment right away.This depends upon your suitability on certain other parameters which will be discussed by your doctor.

The investigations vary in rates from one place to another. On an average, the investigations should cost less than 1500 INR and the Retinoid treatment, which will be in the form of visits, may be six to nine times, will depend upon the doctors fees. There are no additional charges except in case where some procedures are advised. All cases do not need procedures. However, treatment for scars and marks generally requires procedures and is usually done after Retinoid course is over.

In general, the treatment lasts 10 to 14 weeks but variations might occur.
