Brief Answer:
Mainly diet modification.

Detailed Answer:
Hello dear,welcome to HCM forum.

Piles is formed by dilatation of venous channels in the anal canal.
The treatment is conservative initially. The most important thing is to mainatin a soft stool consistency.Any factors that causes hard stool(like irregular diet timings, fast food items, non veg items,greasy/oily food items etc) will make the condition worse by increasing the venous pressure.
So make appropriate changes in your diet-keep regular timings, consume enough fruits and green leafy veges, drink enough water, reduce the proportion of non veg.
When needed during exacerbations, this can be supplemented by laxative and ,analgesics.
If these measures fails to control the situation, causing recurrent episodes of pain and bleeding, surgery is needed.
You can keep following up with a surgeon.

Regarding pilex,I am not sure as this is a herbal preparation.

Hope this will answer your query.
Wishing you good health.
Thank you.
