Brief Answer:
Circumcision(surgery for phimosis) is the best option, check for infection

Detailed Answer:
Hello Mr XXXXXXX

Thanks for writing to us.

I have seen image. You have phimosis. At age of 35, I guess you have tried manually to get rid off phimosis until now but no success. So the best option is go for surgical correction of phimosis. It is called circumcision. A day care procedure and minor surgery. In this your foreskin over the glans(head of penis) will be removed and later on you can do sex normally. It will also prevent chances of recurrent infection.
As you did not have sex yet, there is no problem so far. But with phimosis your sexual experience can be painful in future.

Complains of itching suggests that there is likely some infection. You need to be examined by doctor and if needed urine examination to be done. Medicines can be given after examination as it may be fungal or bacterial.

Best thing is consult a general surgeon and get treatment for both infection and phimosis.

Hope I have answered your query, I will be happy to help further.

Regards,
Dr.Chintan Solanki.

Actually i started having this problem only from last 5 years before everything was good. Do you still suggest surgery or would not be solved with some ointment?

Brief Answer:
Yes surgery is the best option

Detailed Answer:
Hello Mr. XXXXXXX

Duration of 5 yeas is more than sufficient to solve problem manually. If not improved, surgery is the best & overall beneficial option.

If you have not tried to retract foreskin manually until now in five years then only you should try.Consulting surgeon and physical examination can suggest confirm opinion.
