Yeast infection/secondary bacterial infection are possible.

Detailed Answer:
Hi,
Welcome to HCM
Thanks for posting your query.

1.You have this problem ( difficulty in retracting the foreskin) since how long?
2. Are you married or not? If yes how long?
3. Have you any other sexual contact? Oral or vaginal or anal sex?
4. Do you have Diabetes Mallitus?
5. What type of drugs are you on for Psoriasis?
These questions have relevant to your problem and the details would give more value to arrive at the diagnosis of your problem.
The picture sent by you revealed that you are having inflammation of the foreskin of penis, possibly due to Yeast infection.
Diabetes and partner infection have to be corrected along with the treatment of yeast infection with anti-fungal topical and systemic drugs.
The liquid collection underneath the foreskin is due to secondary bacterial infection due to inability to clean the glans penis and inner aspect of the foreskin. Proper washing and antibiotics and anti-microbials are necessary to correct this problem.
Consult a Dermato-venereologist, he/she will easily cure your problem.

Your difficulty in retracting the foreskin is of short duration. It is due to yeast infection and bacterial secondary infection.

Brief Answer:
Anti-fungal antibiotics and Metranidazloe have to be added.

Detailed Answer:
Hi,
Welcome back.

It is better to consult a Dermato-venereologist or a Genitourinary physician rather than a urologist for your problem.
The margin of the foreskin of your penis seems to be soddened. This is common with yeast infection. This needs antifingal antibiotic topical and oral preparations for 3 weeks.
The fluid collection underneath foreskin needs some Antibiotics (preferably related to penicillin), Taxim can be tried along with Tablet Metranidazole against anaerobic organisms.
Proper cleaning techniques of underneath of foreskin when it was not able to be retracted must be taught correctly to facilitate an early cure.
