Suggest Treatment For Persistent Sore Throat And Infection

Question: Hello Doctor,
On 20th Midnight, i had Sore throat and so immediately visited Doctor in the Morning. He prescribed medicines for 7 days but Consecutively for 2 days i was not able to sleep after taking prescribed medicines. I went to doctor on the third day as i did not had any relief from those medicines.

Doctor advised me for Endoscopy for Throat and after it was completed, he said am suffering from severe Throat infection which has so many red patches all inside my throat. He increased the dosage of the same tablets for 10 days along with New tablet ( Glucort 6mg) and the Mouth wash.

I finished the Course but now again it started with throat pain and am really help less on the Throat pain am facing now. Now it started with cough and daily am not able to sleep during night times due to cough. I get some kind of irritation in the throat and i had to cough so not able to sleep whole night.

Please provide me proper medication. i will attach the prescription for your reference.

5.Are you suffering from fever or cold like nasal blockage or nasal discharge?
6.Is any pus like yellow discharge coming from your throat or nose?

Below are the details inline:

1.Is the cough productive(means phlegm coming out) or is it a dry cough?
* It is dry cough but yes sometimes if i cough very hard, little phlegm comes
out.
During night times, i feel irritation in the Throat and had to cough very badly but
no phlegm comes out. Only during morning times after i wake up, i had to spit some
phlegm daily. Cough happens during night times more.

2.What is the intensity of your throat pain:mild/moderate or severe?
* Now it is mild and when i try to swallow any solid or liquid, i get the throat pain.

3.Has the throat pain reduced after taking medicines and again it recurred?
* When i was taking the medicines, i could see some improvement but after the course
is completed, now it again recurred with throat pain and could feel little phlegm
coming out when i try to cough.

4.Are you suffering from change in voice?
* No

5.Are you suffering from fever or cold like nasal blockage or nasal discharge?
* Slight Fever and yes, nasal blockage happens for me daily in alternative nostrils. For
example some times my right nose gets blocked and other time my left nose gets
blocked. When i sleep during night times, one nose will get blocked always.

6.Is any pus like yellow discharge coming from your throat or nose?
* Yellow discharge comes out from Throat. When i wake up in the morning, i had to
spit little yellow phlegm daily.

You might need to start other Antibiotic

Hi XXXX,
Thank you for your follow-up reply.

Looking at the information given, you still have inflammation in your throat and upper respiratory tract.Bacterial infection might still be there.The dose of Antibiotic which you have taken(XOP-CV 325 ) which contains Amoxycillin with Clavulanic acid, seems low dose according to your severity of initial throat inflammation. In most cases, the dose is 625mg two times a day.

You may get relief by taking the following medications:

Oral Antibiotic like Levofloxacin(500mg), once a day for 5 days,
Analgesic with Anti-inflammatory like Ibuprofen,
Antacid like Ranitidine,
Antihistaminic with Decongestant like Levocetrizine and Phenylephrine,
Cough expectorant like Ambroxol,
Paracetamol, in case of fever and
Nasal Decongestant drops like Oxymetazoline.

Also, do steam inhalation.
Do warm saline gargles.
Drink lots of warm fluids.
Avoid cold, spicy, oily food.
Warm turmeric milk will also give relief.
Hot tea with lemon, ginger and honey will also give you relief.
Avoid dust.

I will start the course you mentioned but can you please kindly be more specific:

1)Levofloxacin(500mg) Once a day for 5 days ( will take this tab after Break fast)

2)Ibuprofen --> Is it for 5 days only? if so, how many times do i need to take in a day.
please advise.
3) Ranitidine --> Is it for 5 days only? if so, how many times do i need to take in a day.
please advise.
4)Levocetrizine/Phenylephrine -> Is it for 5 days only? if so, how many times do i need
to take in a day. please advise.
5)Ambroxol --> Is it for 5 days only? if so, how many times do i need to take in a day.
please advise.

Also i bought Otrivin Nasal drops yesterday night. Can i continue using it or you want me to replace with Oxymetazoline.

The dose and duration of the medicines would be as follows:
1.)Tablet Levofloxacin(500 mg)
once a day after breakfast or lunch
2.)Tablet Ibuprofen(200 mg)
two times a day(after meals)
3.)Tablet Rantac(ranitidine)
two times a day before meals
4.)Tablet Levocetrizine 5 mg with Phenylephrine 10 mg
once a day during night time
5.)Syrup Ambroxol 30mg/5ml
two to three times a day
6.)Continue using Otrivin nasal drops
3 drops three times a day

Take all the medicines for 5 days.
