Brief Answer:
?Perianal / Chronic Buttock Abscess / Under Evaluation

Hi Dear !! Thanks for your query with attached picture to HCM.
I reviewed facts of your query in context to your health issues.

My opinion-

Dear,in the given situation-You seem to have perianal Abscess with un-diagnosed fistula in ano, which needs further investigation.

The recurring abscess could be just- boil with chronic abcess-or cause of its recurrence could be-hidden fistula in ano with abscess presentation/ or it could be Pilonidal sinus, which needs to be ruled amongst the various possibilities for such Buttock Abscess

Do's Suggested-

For this I would suggest you to Consult Surgeon, who would do needful physical and perianal checks with investigations.

CT Scan -if needed , after proctoscopic examination, would resolve the extent of the fistula / or perianal abscess/ or Ischio-rectal abscess in your case.

Surgical Incision / drainage with Abscess wall scrapings-

If it is just a chronic incompletely drained boil on buttock-Surgical drain with scrapings of abscess wall under antibiotic cover, would cure it once for all.
Diabetes needs to be ruled out as a cause of recurring abscess.

Remedy for Non-responding/ recurring abscess-

If it still recurs-and proctoscopic examination with CT pelvis to study the anatomic connections of this recurring abscess would be needed to plan further treatment by-fistulotomy with scraping and stent- if its of high variety.

Antibiotic cover with antiprotozoals / and ruling out fissure in ano and ruling out
Crohn's disease would be needed before planning any major surgical treatment as suggested above.

If you update more information/ details upon these suggestions,I would give further suggestions on medicines to correct your health issues.
