Suggest Treatment For Persistent Pain In Perineal Region

I had red pustules in the glans area almost 20 years back. it was recurrent and used to go on applying clotrimazole. then one urologist put me through a month course of antibiotics and the pustules got cured. however since then I have intermittent pain in the perineal region. I have had several KUB ultrasounds, prostate ultrasound, semen culture and dre but all appear normal. I am now suffering from frequent urge to urinate. also there is pain in the perineal region also. sir could it be some urethral stricture or problem with prostate. doctor has prescribed alfuzocin 10 mg and soliten 5 mg for one month. I started first dose yesterday. does this treatment have any side effect and also do we have to take these drugs permanently or they cure too and can be stopped. regards

Hello xxxxxxxxxx and welcome to HCM.
As an Urologist, i can understand your concern.
The main symptom bothering you at present, is frequency and urgency to urinate.
The commonest cause for this is urinary infection (UTI ).
1. Get your urine routine and culture test done.
If active infection is present, treatment will cure your symptoms.
2. Also check your blood sugar,creatinine, and PSA.
That will screen for, diabetes, kidney function and prostate gland changes.
3. Urine flow rate is needed, to record the pressure of urine flow accurately.
4. Though urethral stricture is unlikely, depending on the response to treatment,
it'll help to decide if, a cystoscopy is needed.
Alfuzosin can cause lowering of B.P.,thus this is advised to be taken at bedtime.
This side effect occurs in very few persons, and is seen in the initial few days.
You will need to review the response to treatment, with the Urologist, and accordingly decide on, how long you'll need to take the medicines.
Solifenacin may be stopped after a short duration.

i have had urine culture and routine checked. but there it shows to be sterile. 2 why do i have this slight heaviness and nagging pain in the perineal region. blood sugar, creatinine and psa have been normal in the past tests, however i will get it rechecked. ultrasound showed normal prostate size
2. urine flow is slow in uroflowmetery test. i will get it done and update reports for your consideration. as advised i will stop solofenacin after a month.
3. do these medicines help to cure permanently or are symptomatic relief only.
4. in case i want to see a urologist in person do u offer consultancy on a personal level also

You're most welcome to consult me in Kochi.(Silverline hospital,Kadavanthara).
1. If your urine culture was done over a month back, you need to repeat it.
If all your reports are sent,it's easier for me to assess your problem.
The Urologist has prescribed Alfuzosin, because your prostate gland seemed to be the main problem, in his check-up. He wouldn't have done so, otherwise.
2. The prostate gland is the cause for your urine flow to be slow and without force.
3. The prostate gland increases very gradually with aging, like cataract in the eye.
So, if you stop the medication, your symptoms should recur.
4. Perineal pain has to be checked personally, with a physical examination, by a competent Urologist.
Get back with all reports, and any other medications, if you're taking them.

one last question. does cystoscopy also help and also how long it takes for cystoscopy and whether it is done under anaesthesia. is the procedure painful and can it create infections also. also does long term use of alfuzosin create any long term or harmful side effects.

Hi again.
A cystoscopy test helps to exclude causes inside the urethra, for the poor urine stream, like urethral stricture you asked.
It also helps to assess the prostate gland internally.
1. A cystoscopy procedure is done under local anesthesia.
2. According to the experience and expertise of the Urologist, it's usually done as a o.p.d./ day case procedure.
3. Unless your culture report is seen, cystoscopy shouldn't be done, in case of UTI.
4. As in my previous reply, you need to have a regular review with your Urologist, while on treatment. No major side effects are seen in majority.
