Question: My Mother Age 65, Numbness in both legs and hands. No diabetes.
numbness is persistent for one year.

What is the reason and treatment?

I can understand your concern. The numbness in all extremities is suggestive of "glove stocking type sensory involvement". It is a common feature of "peripheral polyneuropathy". There are so many reasons of neuropathy in a 65 years old female. We have to investigate her and treat her accordingly. The causes and treatment of each cause is given below. Kindly go through it and help your mother

: Diabetes is very common cause. It has already been ruled out. My advise is - get her blood sugar examination monthly to rule out any future possibility.

: Vitamin or nutritional deficiency - Deficiency of B12, folic acid, pyridoxine can cause it. It usually presents with other features like mouth ulcer, dryness, burning in feet, weakness, poor immunity to infection.

Management includes confirmation by measuring of B12, folic acid level. It can be treated by
; Injection B12 - Usual dose of 1500 microgram is given daily for 10 to 15 days after proper prescription.
; Tablet folic acid 5mg can be taken after proper prescription.
; Multivitamin capsules containing pyridoxine, pantothenic acid, vitamin E can be taken daily after proper prescription.

: Drug intake - Various drugs can cause neuropathy. If your mother is taking any medication on regular basis, kindly let me know to rule it out as the cause of her symptoms. Common drugs which can cause neuropathy are metronidazole, isoniazid etc.

: Chronic illness - Sometimes many chronic illness present as neuropathy only like diabetes, amyloidosis, paraneoplastic syndrome of cancer, chronic metal toxicity like lead(from water pipes/paints/polish/Sindoor), mercury(from water/air pollution).

It can be suspected if all other causes are negative. Through history and examination, ultrasound abdomen, chest X ray, lymph node examination and blood investigation are needed to confirm it. You should consult doctor for it. Further treatment will depend upon reports.

At home, you can follow few tips

: Give her balanced diet which include milk, curd, green leafy vegetables, fruits, multigrain food. If non vegetarian, she can eat eggs, red meat and fish.

: Maintain proper hydration

: Observe any wound or burn on her extremities and if present, it is a serious sign. It suggests that she is having sensory loss. Ask her to wear gloves to protect her hands, wear soft shoes, avoid hot/warm place like kitchen/oven etc.

: You can give her multivitamin as mentioned above.
