Question: Hello!My name is XXXXXXX I'm a 35 yo female going through some horrible dizziness for the last five months.Ive had numerous CBCs...(vitamin/mineral levels are fine as well as all organ function).Two MRIs to rule out MS on brain and neck.EEG was normal,EKG was also.Went from neurologists to cardiologists where I wore a heart monitor for 30 days and all was fine.I quit smoking after 17 pack a day years 9 months ago,the symptoms however started about three months after that.The dizziness has made me collapse(not lose consciousness though)and is normally accompanied by pins and needles.Im eating about 1400 calories a day as well as working out(I teach aerobics or did until this ailment got the better of me).Im a waitress and very active,the dizziness only hits while I'm active.My blood pressure rises when I stand and is sometimes slightly high.I also has a tilt table test that was normal.Blood sugar is normal and I'm very health conscience.Im in medical debt as well as missing my old life.Im turning to a clairvoyant next because I'm at a loss.Please,if you have any suggestions I'd love to hear them.

I should also point out that my ears have been checked.Do you think that this could be some freak side effect (months later)from quitting smoking?I actually felt fine for months other than cravings.

Dear madam
1. If your blood pressure has been checked in sitting and thereafter in standing position ? This is important to rule out orthostatic hypotension.
2. If there is no apparent ear, cardiac and neurological problem, a generalized cause like low hemoglobin and disorder of eye should be ruled out.
3. Relation to smoking is unlikely.
4. Do let me know the details. When you get dizziness, how long it lasts and how is it precipitated and relieved?

The dizziness hits me once I've been up and active for about 2 hours.Its not every day like it used to be but the intensity is as severe.The episodes can last for 5-20 seconds.I break out in a sweat and feel the need to hold onto something while taking an order or walking from table to table.One time after teaching a step class I collapsed and was disorientated for a couple minutes but I was aware of my surroundings.Its a very brief moment of dizziness,confusion and sweat and it can happen as many times as 7 per day.I don't feel good on the days it happens.Resting pulse rate is usually around 86,BP can be 146/93 on severe dizzy days some times 140/82 is the norm(cvs has a BP cuff next door to where I work).

Dear madam
Your detailed history points towards dysautonomia i.e. dysfunction of autonomic nervous system. By checking BP in lying and upright posture I meant looking for postural fall in BP. I would recommend detailed neurological examination for autonomic dysfunction all such patients of mine.
Beside, a thorough eye examination and general biochemical profile will be on cards.
Hope this evaluation will pinpoint the cause of your symptoms.
