Question: Hi Doc,
I got Diarrhea almost 2 month from now, it started by eating very hot spicy food few month ago, and since then I got diarrhea and watery stool.

But all of that without other synthoms ( no fever, no rash, no swollen lymph node etc )
I only feel discomfort in my stomach ( bloating, and lots of gas that i am often burpping and fart ), I also hear sound like moving gas trough my stomach

I got blood tested and everything was normal, no anemia and etc, but my widal slide showed that parathypi AO and BO is 1/320, Salmonella IgM is 6, but my CRP is 0.08, and my CEA is 2.18, also my Helio Bacter test is normal

My feces test also normal, no amoeba, no occult blood, no blood, or anything else

I also got Abdominal USG and all is normal and nothing is abnormal

My best friend ( he is a neurologist doctor ) said that i got gastritis

I had anxiety disorder few years ago but now I am cured, but yes lately i am in stress condition

My question is:
What do you think it sound like about my condition? Is it true I am have gastritis? Or I am IBS?
I am worried about my change of stool to watery and but sometimes it is become hard again
My frequency to the toilet is about 2 times or 3 times a day and i have no pain or fever or fatique or anything else

My mind keep telling me to do endoscopy and colonoscopy but my friend said that is no need, he said i just stress and need nothing to worried

Please help me with your oppinion about my condition,
Thank you

Brief Answer:
Most probably IBS

Detailed Answer:
Hello

Thank You for contacting HCM. My name is Dr Muhammad Ali Rauf & i will look into your problem.

I have gone through your query and would try to help you in the best possible way. My provisional diagnosis in your case will be Irritable Bowel Syndrome(IBS) secondary to stress induced. IBS is mostly caused by stress & anxiety. There is absolutely no need to worry. Combine following general & medical advise and you will improve:

1. General:
> Stress is in everyone's life but if it overpowers you then it will start causing problem. So try to remain calm and stress free.
> Go out for a walk in the evening. It will reduce stress & make you fresh.
> If stress is severe then you can visit a psychologist for "Cognitive behavioral therapy" who will guide you relaxation techniques.

2. Dietary Advise:
> Take high fiber diet (peas, beans, broccoli, etc) including psyllium (ispaghul hask)

> Avoid:
* fatty food & caffeine products
* Spicy food.
* Fruits & salads for the time being.

3. Medical Advice:
> Loperamide 2 mg orally thrice a day

Most probably colonoscopy and/or endoscopy will not provide significant results BUT for your personal satisfaction you can get it done.

Hope I have answered your query. If you have any further questions I will be happy to help if not then don't forget to close the discussion and rate it.
Wishing you good health !!

you are most welcome. i'll be available 24/7 for if you need any assistance.

you can take medicine for 7-10 days depening upon the prognosis. IBS usually takes some time to resolve if proper care is taken in all aspects, specially stress. you can see positive effects in 1-2 weeks.
