Suggest Treatment For Perioral Dermatitis

Question: My grandson has an unusual condition of his lower under-lip (that part of his outer lip that is actually below the lip, but still considered part of the lip and upper over-lip. Both of these, but especially the lower part suffer from a kind of chapped-ness. We have tried moisturizers, prescribed topical antibiotics, more moisturizers -- nothing works. The skin will heal to a point of near healthiness: scabbing virtually (but not completely) disappears over the course of about ten days, the skin color is preternaturally pink, making it appear as though his lower lip was extending down toward the beginning of his chin. At this point, the condition will worsen even though we are maintaining whatever treatment has gotten him to this point of recovery. The course of this problem makes it appear as though it was some form of topical MRSA. I've taken him to a dermatologist who blamed in on over-biting, but after constant watching we have determined that is not what is happening. His GP thought it was an infection -- hence the topical AB; but that didn't work. So now I'm looking for other suggestions.

I have gone through your complete query and noted your concern. After diligently analyzing the picture and correlating with the details let me guide you that it appears to be simple Lip licking dermatitis.

To tell you elaborately it can be due to any minor friction or abrasion over the lower lip area. As the lip area is quite soft so even a single time licking over the affected arwa makes the area prone for further damage in children.

I can appreciate that you have observed that the child is not over biting. But I would like to guide you that mere licking or touching the lower lip with tongue could cause it. The saliva in such a sensitive skin could irritate the skin. I agree with your treating dermatologist but jot by biting rather simple licking or touching only.

I suggest you to meet your dermatologist again and get proper treatment for this. There is no point of MRSA in the same so don't worry about the same.
