hello doctor.. please find attached the pic of my penis! the foreskin is tight and hasn't gone back to it's original position. I consulted a dermatologist and he asked me to apply a couple of lotions! I ve applied it thrice already and there is no much improvement. please help doctor. what should I do? should I go ahead with the lotion and wait for some more time or should I go to the nearest dermatologist again? it doesn't pain much but when I touch it, it does. it's a weird sensation!

Welcome again and thanks a lot for your swift follow up with details.

Keeping your situation on most priority I have reviewed your picture and can make out that there is mild to moderate swelling is present. Though you have not told the exact time period but in my patients I always take it as "Medical emergency" situation and treat it at the earliest.

As in paraphimosis the skin around the glans penis acts as band and narrows the blood supply so it is always best to get it treated at the earliest.

Applying lotion will not help in your kind of cases and I always prefer to retract the skin manually as early as possible. Sometimes if the skin is not retracted or if there is delay of multiple hours then I prefer to refer the case to a surgeon colleague as mild surgical manipulation might be required.

In your situation it is best to meet a surgeon at the earliest to get the skin reposited back to normal position. "Do not wait and get the skin reposited at the earliest".

thank you so much doctor. I wish you the same ! I have an couple more questions for you. The surgeon who fixed my problem has asked me to get a circumcision done because he thinks that this situation might occur again in the future. but I am scared of the pain that it can produce post operation. could you suggest me what I should do? whether I will be able to enjoy sex post operation , how much the penis head will be exposed and what are it's repurcussions?

Your surgeon is correct in his view that you might get recurrence the condition because of narrow prepuce.

In such kind of condition circumcision is often one of the best method. This is medically proven to be best as prevents any chance of recurrence and never hers with your Sexual function rather some patients reports increased Sexual potency.

The only issue of religion in many cases of Hindu origin. In Hindu population is not considered to be a good thing as it exposes the complete glans Penis.

It is not painful as is very minor procedure. So do not worry.

As I said previously that it is medically proven to be good and protects from multiple infections too. So you can take decision accordingly.

thanks doctor. one more very important qs.. I am little embarrassed to ask too. a couple years ago, I was infected with genital warts because I had oral sex with an unknown female. I consulted a dermatologist and got it removed. 4months ago , it started growing again but it was jus like a very small mole which didn't grow much so I left it at that. just couple of days back, I had unprotected sex with my girlfriend. I am feeling guilty that I might have passed on the virus to her. could you please help me , what I should do in this situation? can I get rid of the warts once and forever!! can she be prevented from getting the warts ! please help doc.

Regarding genital warts: these are one of the most simple sexually transmitted infection and are one of the easiest treatable condition.

To relax you more these are one of the commonest STD seen. So first and foremost relax your mind for anything serious regarding genital warts.

Now as you had been treated for warts almost 4 months back so it unlikely to transmit to your girlfriend. Most often warts occur and recur within 4 weeks of time.

As it has been more than 4 months so just relax for any issues. There is no need of any treatment for you as well as your girlfriend in this case.

But it is my personal suggestion to you that till the time you are not in a committed relationship, you should always use protection for your as well as your partner's safety.

I always suggest my patients to have safer sex practice in form of condoms to relax the mind from any STDs.

I hope you must be relaxed to know after knowing all these simple facts. I will be glad to guide you any further.
