Brief Answer:
Could be vitamin deficiency/fungal infection

Detailed Answer:
Hi
Welcome to Healthcare-Magic
Greetings of the day

Dear Ms

Painful red lesion over the tongue of child could be possibly due to two main factors

1. Vitamin deficiency especially Vitamin B2

2. Oral Candidiasis( fungal infection)

Exact Diagnosis will however depend on good clinical examination. It would be very helpful if you could send me clinical photograph for further evaluation.

In the meantime , Oral Multivitamin supplement and oral nystatin drops( anti fungal) will be helpful

Awaiting your response

Take Care
