Question: My daughter noticed a painful pea sized lump in the end of her tongue yesterday. She can not see it but can feel it with her fingers.

Thanks for your query.

I understand that this is a query for a daughter who is 20 years old.
That she noticed a painful pea sized lump in the end of her tongue yesterday.
That she can feel it with fingers but can not see it.

I would like to get a clarification as to whether you are talking about the tip of the tongue or the back part of the tongue towards the throat ( at the end of her tongue )

As the patient is young girl the probable causes of such a painful lump in the tongue ( tip) are as follows:
What we call as Foreign body granuloma. This can be due to a small particle of food getting stuck inside the tongue and the tongue reacts to form a painful lump within. This is body's normal reaction.
Another rare cause is inadvertent bite that was unnoticed.

If the color over the lump is normal, she can take some anti-inflammatory medicines like Ibuprofen and the problem may be solved. Examination by a Doctor and an antibiotic if the signs of infection may be needed.
Most of the other lumps of the tongue are never painful and are benign at this age group.

As said the commonest reason in young people is a granuloma. This is painful and more importantly very irritating to the patient.

If the color over the lump is normal, she can take some anti-inflammatory medicines like Ibuprofen ( if suits her) and the problem may be solved. Examination by a Doctor and a oral antibiotic may be needed if the signs of infection develop. Local applications do not help much.

She has also recently started wearing an upper mouth piece all the time for TMJ problems. Could this be irritating it. She noticed it yesterday after she had an adjustment to the mouth piece. She has it adjusted about every six weeks. Just wondered if it could cause lump or irritated her tongue.

Certainly the upper mouth piece can irritate it, and as per your history the adjustment was done recently ( same day) Irritation can cause a painful lump as the tongue is highly muscular organ and responds very fast as it has very high blood supply, but this helps for early resolution too.

I hope her problem is solved very soon.
