Brief Answer:
Most probably polymyalgia rheumatica

Detailed Answer:
Hello

Thank you for getting back with the answers. There could be two probable causes in my opinion:
1. First, polymyalgia rheumatica which is an inflammatory condition with unknown etiology that affects elderly person. In this condition there is pain in shoulder region, more pronounced in the morning but becomes relieved with the passage of day. For this you need to visit hospital and take an appointment for a rheumatologist. He will examine you in detail and then will prescribe treatment. This condition is self limiting. To prevent pain and stiffness low dose steroids are treatment of choice which causes a near complete resolution.

2. Second, cervical rib compression. In some individuals there is an accessory rib in the cervical segment which compresses particular nerves, as a result it causes pain in that particular shoulder region up til hand and the hand gets sleep sensation. This can be ruled out by undergoing an X-ray of cervical region.

Hope I have answered your query. If you have any further questions I will be happy to help if not then don't forget to close the discussion and rate it.
Wishing you good health !!

Brief Answer:
There are tests that represent inflamation

Detailed Answer:
Hello

Thank you for getting back. There are some tests like CBC with ESR and C-reactive protein which help to indicate ongoing inflammatory process. Similarly MRI or ultrasound of joint may show inflammation of tissue. Results are then correlated clinically. There are no specific test that will pin point it.

Hope I have answered your query. If you have any further questions I will be happy to help if not then don't forget to close the discussion and rate it.
Wishing you good health !!
