Question: shoulder blade pain under ribs been going on since january of 2015.
pain is more of an annoyance.
have had lots of gas and stomach rumblings.
unexplained weight gain and more unusual is inability to lose weight, despite being an avid runner, ran 2 20k races this spring and living a very active life.
constipation.
also lethargic.
have mood swings.
tough getting out of bed because of fatigue.
i've been unmotivated at work and at home.
i feel something is amiss.

Welcome
I have gone through your query.

Gastric discomfort, weight gain can be responsible for causing referred pain in shoulder region.
First we try to address the stomach problem, and observe for any improvement in shoulder pain.

I would suggest you few measures that will be beneficial.
Take capsule omeprazole 20 mg once daily empty stomach, before breakfast for a week.
Avoid spicy and oily food for few days till the symptoms are resolved. Avoid or reduce the intake of meat, dairy products, and add more fiber to your diet. Eat lots of salads, vegetables, fresh fruits.
Avoid excess consumption of tea/ coffee/ hydrogenated drinks. Avoid smoking and alcohol, if any.
Eat frequent small meals instead of heavy ones and do not starve. Take plenty of water.
Eat light meals prepared under hygienic conditions. Avoid eating fast and talking while eating.
Regular exercise is very much essential. Brisk walk every day for 30 minutes in the morning and then slowly increase the dura if time permits. Follow this measures your symptoms should reduce.

Try this measures for week , if this is not of much help then , I would suggest you to go for ultrasound abdomen to rule out other pathology like gall stones.

perhaps i was not as clear as i wanted to be.
the "pain" is more of an annoyance and not really pain and not in the shoulder.
it is under the shoulder blade on right side and then down the rib cage.
upper right back in rib cage.
what would explain the weight gain, the fatigue, lack of motivation?

can i buy omeprazole 20 mg at a local store?
does it require perscription?

Prilosec OTC is available as over the counter agent , kindly ask for it in local drug store.
