Suggest Treatment For Pain In Hip Due To Ligament Tear In Thigh Area

Question: My 79 year old man friend has been having food poisoning symptoms, and has been told he has a ligament torn in his thigh area. As he also has pain from his hip to his knee. This man is unusually strong and has never had illnesses until the last 2 years. He has had 3-4 bouts of this stomach problem: vomiting, dry heaves, pain, and a CAT Scan did not reveal any issues. Also has had some constipation. He just last week got over his stomach issue & today it started with breakfast & walking. I suspect he eats old food, but not this time. Help! I am now in a different state.
He also had an MRI last week and nothing unusual showed up from what he said. He has had colonoscopy and the Dr. said his plumbing was unusual. Any thoughts?

I have gone through your query with diligence and would like you to know that I am here to help. As there are two separate issues you have discussed with the elderly gentleman you would like to help, I shall address them separately.

Starting with the ligament rupture or strain. If at all his pain is in the anterior or front side then this has to do with the quadriceps muscle or its tendon, and if it is in the posterior or back side then it has to do with his hamstring muscles. Both these muscles are quite strong and are also quite prone to injury especially in sports persons and those who are quite active. If he can still lift his affected leg up and leave it down slowly, then the tendon is not completely torn or ruptured. And if at all the tendon is not torn completely then the treatment is as follows:
-Rest
-Ice pack to the affected region
-Compression: Using a soft elastic bandage
-Elevation of the limb to prevent further swelling
This process of healing is natural and might take some time, so please request him to be patient. If at all, after 2-3 weeks of strict rest he still complains of pain then surgical intervention might be necessary.

As for the stomach issue, as you must already know symptoms can only tell us externally that there is some issue internally that needs visualization. For this reason a colonoscopy would have been the most appropriate investigation, and without the reports I cannot really say much other than providing you with a list of possible conditions. I shall provide you with a list of possible abnormalities, but I would also request you to speak to the gentleman in discussion and request him to pass on his colonoscopy reports to us, so I can study them and tell you what exactly is wrong.

For now, here is the list of possible conditions ma'am:
-Irritable bowel syndrome (Crohn's disease or Ulcerative colitis)
-Inflammatory bowel disease
-Diverticulosis
-Intestinal polyps
-Gastritits
-Pheochromocytoma
-Colitis
and a few others ma'am. As I mentioned above, without visualization through various investigations it is close to impossible to reach an accurate diagnosis.

If he has already had a colonoscopy & endoscopy (I'm not sure about this one) what other tests would be useful to try to identify the problem. This nausea & vomiting with dry heaves makes him bedridden.

If it were anything to do with a growth, obstruction, tumor, cancer, abnormalities leading to a structural defect; these would have been seen on the CAT scan and/or MRI scan. But as these have returned normal, we have to suspect an internal chronic condition like the ones mentioned in my previous response ma'am.

Which is where an endoscopy and colonoscopy come in. If only we had the results of these investigations, which should have reported some abnormalities; we can surely assist him further.

These are the top 4 investigations used to find the cause for his presentation, and as his doctor reported some abnormality after observing the colonoscopy I would like to read his/her report to learn more about the condition, which then can be followed only by treatment and god-willing, improvement to a point of complete recovery.

If at all none of these investigations have showed much, then the ultimate investigation would be a PET scan, but there is no indication for this as of now, and moreover as the colonoscopy has already found some abnormalities, we can just work with those results for now.
