Suggest Treatment For Pain During Urination

Question: Hi,
I'm 24, Male and contracted chicken pox symptoms on body since 17th May. I've painful urination and nerve pricking at the end of urethra since 18th May. I've been taking 600Mg Aculivor (Herpikind) since 18th Night. I also take levocetrizine once a day. The pain in urethra has increased and today morning I had a painful nighfall. My itches worsen at night. Please tell me what to do for the new issue that arised.

I carefully read your question. Normally chicken pox does not cause urethral symptoms. Do you have lesions all over the body or only on the genital area?
If you have symptoms spread in the body very likely you do have chicken pox and in this case the urethral symptoms do not seem to be related to the chicken pox.

If you have lesions only in the genital area and very likely you might have shingles or herpes simplex and in this case you might get urethral lesions and symptoms.

Acyclovir is a drug that should be taken several times a day. For example 200 mg 5 times a day or 400 mg 3 times a day. If you are taking only 600 mg a day this is not a good enough daily dose.

If your lesions are only in the genital area than very likely this is herpes simplex infection which is considered also a sexually transmitted disease. Even if you have lesion all over the body than you might have chicken pox that is not a sexually transmitted disease but the urethral symptoms would have to be investigated for a sexually transmitted disease so in both cases i would say that it is good to have some tests to check and diagnose or rule out sexual transmitted disease.
I would recommend you to have urethral secretions checked for chlamydia and gonorrhea (or you can do t he blood tests for those).

If you have not had a check in the last 2 years for HIV, syphilis and hepatitis B I would strongly recommend those tests as routine screening tests when a patient has urethral symptoms.

So to conclude i would say:
1. have some tests to rule out chlamydia and gonorrhea to make sure the urethral symptoms are not related to those
2. routine screening tests for HIV, syphilis and Hep B
3. check with your doctor the dose of acyclovir you are taking. Make sure you drink plenty while on acyclovir as it might precipitate in the kidneys if not flushed well with fluids.
4. Take high dose Ibuprofen to help with pain these days
5. If levocetirizine is not helping enough probably benadryl could be a better option (it might cause drowsiness), there are also local creams to help with the itchiness on the skin (check this with your doctor)
6. skin lesions are prone to infection due to damage of the upper layers of the skin and entrance of the bacteria so if you get areas that get red and swollen or ooze than local complication with a bacterial infection should be suspected and treated appropriately (with local antibiotic cream or oral antibiotics)
7. a urinalysis and a urine culture might be good tests to check urine also
8. Your partners will have to be checked for sexually transmitted disease also

Hello Doctor,
I have lesions all over the body, from head to leg and they are drying. The ones near the groin have disappeared. The itchiness on my genital area has reduced and I'm using calamine solution.

I've attached my urine report with this question. The urine culture report says: No aerobic culture growth of organism after 48hrs incubation

I've stopped taking acyclovir and will continue with levocetrizine as I'm feeling much healthier now. There's still some minor stingy pain while urinating sometimes but not prominent and is occational.

I shouldn't have any STDs due to no mutual sexual activity yet also my tests for HIV and Hepatitis came out negative done within 2 years ago for UTI from kidney stone previously.

I am glad you are feeling much better.

Chicken pox is generally a self-limiting disease and takes around 7-10 days for the patient to feel comfortable. Now the lesions should be dark crusts and healing but have a tendency to remain scar and pigmented areas if scratched so take care not to scratch them, local solution and the oral pill will help with it also.

It is good the tests for HIV and hepatitis was fine and if you don't think you are at risk for the sexually transmitted disease this is good also.

The urinalysis you have attached is within normal limits and the culture shows no growth of bacteria which means it is not a urinary infection that causes the symptoms.

Thank you.

I hope my energy levels will return soon however there's a numbing nerve pain that returns after few days in the end of urethra and goes away with rest. Is there anything I can do to stop its recurrence?

Hello,

Energy will return with the passing of the days. Adults generally have more severe forms of Chicken pox compared to children (when it is more common) and generally take longer to heal.
The pain you are having I would say that you can take high dose of Ibuprofen and local cold compresses if pain is only for short periods of time.
Time will be the best treatment and it should improve in the next few days.
