Brief Answer:
Hemorrhoids most likely. Strangulation can cause more pain.

Detailed Answer:
Hi

Thank you for asking HCM.

I have gone through your query. From your presentation it looks like hemorrhoids. It is usual to get hemorrhoids in summer due to dehydration. A strangulated hemorrhoid can cause severe pain also. If such severe pain is there a consultation with a general surgeon is well appreciated. This should be managed surgically in such case.

As it is a second episode it is better to get a digital examination by a general surgeon and to undergo proctoscopy or colonscopy if needed.

You can do sit baths, taking over the counter analgesics like ibuprofen. Hemorrhoid creams like proctosedyl containing proctosedyl and cinchocaine will be helpful also. You should also take more plain water. Take regular exercises like jogging. You can also ayurvedic over the counter stool softeners like softovac which is not habit forming. If still constipation is encountered then prescription drugs like biscodyl suppository or oral will be needed.

Hope this may help you. Let me know if anything not clear.
Thanks.
